THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show the Defense Superior Service Award and the Army Superior Unit Award (4th Award).

2.  The applicant states, in effect, that he was awarded the Defense Superior Service Award and the Army Superior Unit Award (4th Award) and desires them to be added to his DD Form 214. 

3.  The applicant provides copies of certificates for his Defense Superior Service Award and Army Superior Unit Award.

CONSIDERATION OF EVIDENCE:

1.  On 31 December 2013, the applicant was serving as a Medical Corps colonel when he was honorably retired by reason of sufficient service for retirement.  He had served 30 years, 6 months, and 7 days of active service.

2.  The DD Form 214 he was issued at the time of retirement shows that he served in Saudi Arabia during the period 20020830 – 20021130 and in Iraq during the period 20080914 – 20090414. 

3.  The applicant’s records show that he was assigned to the U.S. Army Medical Detachment at Fort Belvoir, VA, from 17 August 2009 until he retired on
31 December 2013.  

4.  The applicant's DD Form 214 shows the Army Superior Unit Award (3rd Award).  However, the Defense Superior Service Award is not among the awards listed on his DD Form 214.

5.  Permanent Orders Number 093-01 issued by the U.S. Army Human Resources Command, on 3 April 2014, awarded the Army Superior Unit Award to the U.S. Army Element, Fort Belvoir Community Hospital for the period              14 August 2010 to 13 August 2011.  

6.  Military Personnel Awards Order Number 14-088 issued by Department of Defense Washington Headquarters Services, on 2 June 2014, awarded the Defense Superior Service Medal to the applicant for the period July 2009 to December 2013. His DD Form should be corrected to show this medal.

DISCUSSION AND CONCLUSIONS:

1.  The applicant contention that his DD Form 214 should be corrected to show the Defense Superior Service Award and the Army Superior Unit Award (4th Award) appears to have merit.

2.  The applicant's was awarded the Defense Superior Service Medal on 2 June 2014; therefore, he is entitled to have that award added to his DD Form 214.

3.  The applicant’s unit at Fort Belvoir was awarded the Army Superior Unit Award, on 3 April 2014, after he retired, for a period he served with the hospital; therefore, he is entitled to have his DD Form 214 corrected to show this award, which would be his fourth award.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the Defense Superior Service Award and the Army Superior Unit Award (4th Award) to his DD Form 214.

2.  The Board wants the applicant and all others to know that the sacrifices he made in service to the United States during the Global War on Terrorism are deeply appreciated.  The applicant and all Americans should be justifiably proud of his service in arms.
