THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 31 May 2014 to show he was awarded the Meritorious Service Medal. He further requests correction of his DA Form 4980-12 (Meritorious Service Medal Certificate) and corresponding DA Form 638 (Recommendation for Award) to show the award period as
1 August 2004 through 31 May 2014 vice 1 August 2004 through 31 July 2014.
2. The applicant states the award was received after his retirement and the end date of the award was after the date of his retirement.
3. The applicant provides his DD Form 214 for the period ending 31 May 2014 and a DA Form 638, dated 23 April 2014.
CONSIDERATION OF EVIDENCE:
1. After previous enlisted service in the Regular Army from 28 July 1994 to 6 March 2006, the applicant was appointed as a Reserve warrant officer on 7 March 2006.
2. He was honorably retired from active duty on 31 May 2014 after completing over 20 years of qualifying active service. His DD Form 214 does not show he was awarded the Meritorious Service Medal.
3. He provided a DA Form 638, dated 23 April 2014, which shows he was awarded the Meritorious Service Medal for the period 1 August 2004 through
31 July 2014 as announced in Permanent Orders Number 163-04, issued by Headquarters, III Corps and Fort Hood, Fort Hood, TX, on 3 July 2014.
4. His record contains a Meritorious Service Medal Certificate for the period 1 August 2004 to 31 July 2014, dated 12 June 2014.
DISCUSSION AND CONCLUSIONS:
Permanent orders awarded him the Meritorious Service Medal for the period of 1 August 2004 to 31 July 2014, even though he was retired on 31 May 2014. As such, it would be appropriate to correct his DD Form 214 to add this award. Additionally, his Meritorious Service Medal Certificate and the orders announcing the award should be corrected to show the award period end date as 31 May 2014.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214, for the period ending on 31 May 2014, by adding the Meritorious Service Medal, and by correcting his Meritorious Service Medal Certificate and corresponding orders to show the period of the award as
1 August 2004 to 31 May 2014.
