		IN THE CASE OF:

		BOARD DATE: 14 April 2015

		DOCKET NUMBER: AR20140014181

THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1. Application for correction of military records (with supporting documents provided, if any).

2. Military Personnel Records and advisory opinions (if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1. The applicant requests correction of a DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) pertaining to her deceased husband and former service member (FSM) to reflect his correct Social Security Number (SSN).

2. The applicant states that the FSM’s DD Form 214 dated 2 February 1968 contains an incorrect SSN.

3. The applicant provides copies of the FSM’s DD Form 214, Social Security Card, and Death Certificate.

CONSIDERATION OF EVIDENCE:

1. The FSM enlisted in the Regular Army on 3 February 1965 for a period of 3 years. At the time of his enlistment the SSN that is contained on his Social Security Card was recorded in his records. He completed his basic training and his advanced individual training as a cook and was transferred to Korea for a
13-month tour on 21 December 1965.

2. He departed Korea on 18 January 1967 and was transferred to Fort Sill, Oklahoma where he remained until he was honorably released from active duty (REFRAD) due to the expiration of his term of service (ETS). The DD Form 214 issued at the time of his REFRAD was prepared to reflect a SSN beginning in the first three digits of “444” instead of “448”.

3. The FSM again enlisted in the Regular Army on 30 December 1970 for a period of 3 years using the SSN contained on his Social Security Card and served his entire enlistment at Fort Hood, Texas. He was honorably discharged on 12 December 1973 and his DD Form 214 issued at the time of his discharge reflects his correct SSN.

DISCUSSION AND CONCLUSIONS:

1. The FSM enlisted and served his entire enlistment under the SSN that is contained on his Social Security Card. However, it appears that an administrative error occurred at the time his DD Form 214 was prepared that resulted in an incorrect SSN being reflected on his DD Form 214 dated
2 February 1968.

2. Accordingly, his DD Form 214 should be corrected to reflect the SSN that is reflected on his Social Security Card.

BOARD VOTE:

___x____ ___x____ ___x____ GRANT FULL RELIEF

________ ________ ________ GRANT PARTIAL RELIEF

________ ________ ________ GRANT FORMAL HEARING

________ ________ ________ DENY APPLICATION

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by showing the SSN that is reflected on his Social Security Card on his DD Form 214 dated 2 February 1968.

In Special Orders Number 34, the fifth digit of his SSN is reflected as 4 and the same is reflected on every document in his official military record except for his DD Form 214. The applicant contends that the SSN reflected on the FSM's DD Form 214 should be corrected. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 to reflect the fifth digit of his SSN as 4 instead of 9 as currently reflected.

The applicant requests correction of his records to show his Social Security Number (SSN) as "XXX-XX-X4XX" instead of "XXX-XX-X8XX." However, during his processing for enlistment into the Regular Army, his SSN was recorded in his military records as "XXX-XX-X4XX." The available evidence shows the applicant's correct SSN was entered in his military records during the period of his active duty.

The applicant requests correction of the DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) of her late husband, a former service member (FSM), to show his social security number (SSN) as "3XX-4X-2XXX" instead of "2XX-3X-9XXX" and that he served in Vietnam. His military records contain the following: * DD Form 4 (Enlistment Record – Armed Forces of the United States), dated 23 February 1962, which does not list his SSN * DA Form 41 (Record of Emergency Data),...

The FSM's military service records contain a DD Form 4 dated 10 April 1965 showing the FSM's Service Number, not his SSN. The FSM placed his signature on the DA Form 1361 (Recommended Findings of Physical Evaluation Board) certifying the information was true and correct. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: a. showing award of the Korea Defense Service Medal; and b. issuing a DD Form 215 (Correction to DD...

His records contain a U.S. Army Administration Center (USAAC) Form 170 (Army Reserve Personnel Data Review), dated 1 March 1967, that shows he attempted to have his military records corrected to show his correct SSN. However, the evidence of record shows the SSN of "XXX-X5-XXXX" was used when the FSM enlisted in the RA in 1962 and when he was released from active duty in 1965. __________X_____________ CHAIRPERSON I certify that herein is recorded the true and complete record of the...

The applicant served in Korea from June 1965 to July 1966 and is therefore entitled to the KDSM.

At the time of his induction his records were prepared to reflect the SSN that is reflected on his social security card. The DD Form 214 issued at the time of his REFRAD shows a SSN containing a “0” as the second digit instead of a “1”. The applicant requests, in effect, correction to Item 3 (Social Security Number) of his separation document (DD Form 214), dated 4 July 1970. The evidence of record shows the applicant’s military records initially listed the incorrect SSN. Therefore, given his attempts to correct his SSN and the fact he used the correct SSN while still on active duty, it would be appropriate to correct the SSN listed in Item 3 of his DD Form 214 to reflect the proper SSN listed on the 1970 OTAG Letter on...
