THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of a DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) pertaining to her deceased husband and former service member (FSM) to reflect his correct Social Security Number (SSN).
2. The applicant states that the FSMs DD Form 214 dated 2 February 1968 contains an incorrect SSN.
3. The applicant provides copies of the FSMs DD Form 214, Social Security Card, and Death Certificate.
CONSIDERATION OF EVIDENCE:
1. The FSM enlisted in the Regular Army on 3 February 1965 for a period of 3 years. At the time of his enlistment the SSN that is contained on his Social Security Card was recorded in his records. He completed his basic training and his advanced individual training as a cook and was transferred to Korea for a
13-month tour on 21 December 1965.
2. He departed Korea on 18 January 1967 and was transferred to Fort Sill, Oklahoma where he remained until he was honorably released from active duty (REFRAD) due to the expiration of his term of service (ETS). The DD Form 214 issued at the time of his REFRAD was prepared to reflect a SSN beginning in the first three digits of 444 instead of 448.
3. The FSM again enlisted in the Regular Army on 30 December 1970 for a period of 3 years using the SSN contained on his Social Security Card and served his entire enlistment at Fort Hood, Texas. He was honorably discharged on 12 December 1973 and his DD Form 214 issued at the time of his discharge reflects his correct SSN.
DISCUSSION AND CONCLUSIONS:
1. The FSM enlisted and served his entire enlistment under the SSN that is contained on his Social Security Card. However, it appears that an administrative error occurred at the time his DD Form 214 was prepared that resulted in an incorrect SSN being reflected on his DD Form 214 dated
2 February 1968.
2. Accordingly, his DD Form 214 should be corrected to reflect the SSN that is reflected on his Social Security Card.
