THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests reconsideration of his previous request for correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 7 April 1989 to show his foreign service as 4 years, 10 months, and 37 days vice 1 year, 11 months, and 18 days.

2.  As a new request, the applicant requests:

* correction of his DD Form 214 for the period ending 7 April 1989 to show he received two awards of the Army Service Ribbon vice one award
* to show on a different copy of his DD Form 214 for the period ending 7 April 1989 the Republic of Vietnam Gallantry Cross with Palm Unit Citation, Vietnam Service Medal, and National Defense Service Medal

3.  The applicant states he first enlisted in the Army in 1964 and he got out in 1968 after being wounded in Vietnam.  He finished his education and then enlisted in the Army and served from 1985 to 1989.  He is displeased that the DD Form 214 he was issued in 1989 does not indicate all he accomplished during his service from 1964 to 1968.

4.  The applicant provides:

* his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) for the period ending 22 October 1968
* two different DD Forms 214 for the period ending 7 April 1989
* DD Form 215 (Correction to DD Form 214 - Report of Separation from Active Duty), dated 6 March 1979
* two DD Forms 215 (Correction to DD Form 214 - Certificate of Release or Discharge from Active Duty), dated 3 May 1985 and 22 January 1993
* four letters, dated between 2 December 2009 and 9 July 2013
* Army Board for Correction of Military Records (ABCMR) Record of Proceedings (ROP), dated 2 July 2013
* Final Decree for Change of Name, dated 22 October 2013

CONSIDERATION OF EVIDENCE:

1.  Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the ABCMR in Docket Number AR20120021978, on 2 July 2013.

2.  As new evidence the applicant provided a copy of his DD Form 214 for the period ending 7 April 1989 that contains different awards in item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized (All Periods of Service)) than the DD Form 214 for the period ending 7 April 1989 contained in his record. 

3.  Item 13 of this copy of the DD Form 214 he provided shows he was awarded or authorized the:

* Army Good Conduct Medal (2nd Award)
* Army Service Ribbon (1st Award)
* Overseas Ribbon (4th Award)
* Aircraft Crewmember Badge
* Driver and Mechanic Badge with Wheel Component Bar
* Purple Heart
* Vietnam Campaign Medal with "60" Device
* Republic of Vietnam Gallantry Cross with Palm Unit Citation (part of this award was mistyped as the "Gallantery Cross Unit Sitation)"

4.  It appears this copy of his DD Form 214 was issued in error.  After he was discharged it was voided and a new DD Form 214 was issued which he was also provided.  Therefore, corrections to this copy of the DD Form 214 for the period ending 7 April 1989 that is not contained in his records will not be addressed further in this ROP.

5.  The applicant enlisted in the Regular Army (RA) on 24 August 1964 and he held military occupational specialty (MOS) 05D (Special Identification Operator).  During this period of service he served in:
* Thailand from 23 August 1965 to 6 March 1967 (1 year, 6 months, and 12 days)
* Panama from 28 April 1967 to 1 October 1967 (5 months and 4 days)
* Vietnam from 15 November 1967 to 18 October 1968 (11 months and 4 days

6.  He was honorably released from active duty 22 October 1968 and was transferred to the U.S. Army Reserve.  Item 22c (Foreign Service) of the DD Form 214 he was issued for this period of service contains the entry 2 years, 11 months, and 24 days.

7.  Item 24 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of this DD Form 214 shows the National Defense Service Medal, Vietnam Service Medal, Vietnam Campaign Medal with "60" Device, Purple Heart, and the Army Good Conduct Medal (1st Award).

8.  He enlisted in the RA on 25 March 1985 and he held MOS 67U (Utility Helicopter Repairer).  During this period of service he served in Germany from 9 December 1985 to 26 November 1987 (1 year, 11 months, and 18 days).

9.  He was honorably discharged from active duty on 7 April 1989.  Item 12f of the DD Form 214 he was issued for this period of service contains the entry 1 year, 11 months, and 18 days.

10.  Item 13 of this DD Form 214 shows he was awarded or authorized the:

* Army Good Conduct Medal (2nd Award)
* Army Service Ribbon 
* Overseas Ribbon (4th Award)
* Aircraft Crewmember Badge
* Driver and Mechanic Badge with Wheel Component Bar
* Purple Heart
* Vietnam Campaign Medal with "60" Device
* Republic of Vietnam Gallantry Cross with Palm Unit Citation
* Vietnam Service Medal 
* National Defense Service Medal 

11.  Army Regulation 600-8-22 (Military Awards) states the Army Service Ribbon was established by the Secretary of the Army on 10 April 1981 (emphasis added).  Effective 1 August 1981, all members of the Active Army, Army National Guard, and Army Reserve in an active Reserve status are eligible for the award upon successful completion of initial entry training.  
12.  Army Regulation 635-5 (Separation Documents), in effect at the time, stated in item 12f, enter the total amount of foreign service completed during the period covered by the DD Form 214.  

DISCUSSION AND CONCLUSIONS:

1.  The regulatory guidance states the foreign service completed during the period covered by the DD Form 214 is listed on the DD Form 214.  The applicant's DD Form 214 for the period ending 7 April 1989 contained in his records correctly shows his foreign service in Germany that was completed during the period covered by the DD Form 214.  There are no provisions for adding foreign service completed during a prior period of active duty to the DD Form 214.  

2.  With respect to the Army Service Ribbon, this award was authorized in April 1981 and issued in August 1981.  His DD Form 214 for the period ending 7 April 1989 correctly shows he was authorized one award of the Army Service Ribbon. 

3.  In view of the foregoing, he is not entitled to the requested relief
