THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of the following documents to show social security number (SSN) XXX-XX-3791:
* DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) with an effective date of 16 December 1965
* DD Form 215 (Correction to DD Form 214), dated 29 June 2006
2. The applicant states his correct SSN, as shown on his social security card, is XXX-XX-3791.
3. The applicant provides his:
* DD Form 214
* DD Form 215
* social security card
CONSIDERATION OF EVIDENCE:
1. Title 10, U.S. Code, section 1552(b), provides that applications for correction of military records must be filed within 3 years after discovery of the alleged error or injustice. This provision of law also allows the Army Board for Correction of Military Records (ABCMR) to excuse an applicant's failure to timely file within the 3-year statute of limitations if the ABCMR determines it would be in the interest of justice to do so. While it appears the applicant did not file within the time frame provided in the statute of limitations, the ABCMR has elected to conduct a substantive review of this case and, only to the extent relief, if any, is granted, has determined it is in the interest of justice to excuse the applicant's failure to timely file. In all other respects, there are insufficient bases to waive the statute of limitations for timely filing.
2. On 27 November 1963, he was inducted into the Army of the United States.
His DD form 47 (Record of Induction) does not show his SSN. At the time of his induction Soldiers were identified by an Army service number.
3. The following documents in his Military Personnel Records Jacket show his SSN as XXX-XX-3791:
* DD Form 398 (Statement of Personal History)
* DA Form 24 (Service Record)
* DA Form 20 (Enlisted Qualification Record)
4. On 16 December 1965, he was released from active duty. Item 32 (Remarks) of his DD Form 214 shows his SSN as XXX-XX-5791.
5. A DD Form 215, dated 29 June 2006, shows his SSN in item 3 (SSN) as XXX-XX-5791.
6. He provided his social security card that shows his SSN as XXX-XX-3791.
7. Army Regulation 635-5 (Separation Documents), effective 13 August 1963, established standardized policy for preparing and distributing the DD Form 214. This regulation stated to transcribe a Soldier's social security account number (SSAN) from the DA Form 24 and enter it in item 32 of the DD Form 214 in the following manner: "SSAN: 000-00-0000."
DISCUSSION AND CONCLUSIONS:
The applicant's DD Form 398, DA Form 24, DA Form 20, and social security card show his SSN as XXX-XX-3791. Therefore, there is sufficient evidence to amend his DD Form 214 and DD Form 215 to show his SSN as XXX-XX-3791.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. amending item 32 of his DD Form 214 effective 16 December 1965 by deleting the current SSN entry and replacing it with the SSN as shown on his social security card; and
b. amending item 3 of his DD Form 215, dated 29 June 2006, by deleting the current SSN entry and replacing it with the SSN as shown on his social security card.
