THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show the award of an Army Commendation Medal with “V” Device.  

2.  The applicant states he received this award after he was released from active duty.

3.  The applicant provides an award certificate and a DA Form 638 (Recommendation for Award).

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 22 October 2008 and, following the completion of initial training, was awarded military occupational specialty 11B (Infantryman).  The highest rank/grade he attained while on active duty was sergeant/E-5.  While on active duty he deployed to Afghanistan twice, from 21 August 2009 to 22 August 2010 and from 21 February 2012 to 29 August 2012.

2.  He was honorably released from active duty on 9 December 2012.  His 
DD Form 214 shows creditable active service of 4 years. 1 month, and 18 days.  This form shows he was awarded or authorized:

* Army Commendation Medal (2nd Award)
* Army Good Conduct Medal
* National Defense Service Medal
* Afghanistan Campaign Medal with two bronze service stars
* Global War on Terrorism Service Medal
* Noncommissioned Officer Professional Development Ribbon
* Army Service Ribbon
* Overseas Service Ribbon
* North Atlantic Treaty Organization Medal
* Combat Infantryman Badge
* Parachutist Badge

3.  The applicant's records contain permanent orders (PO) for four Army Commendation Medals:

* PO Number 024-023, dated 24 January 2010, issued by Headquarters, 4th Brigade Combat Team, 82nd Airborne Division for the period 23 August 2009 to 23 August 2010
* PO Number 321-013, dated 20 August 2012, issued by Headquarters, Combined Task Force (CTF) Fury, Regional Command (RC) South (S), for valor during the period 23 to 25 April 2012
* PO Number 169-338, dated 15 September 2012, issued by Headquarters, CTF Fury, RC (S), for the period 26 February 2012 to 15 September 2012
* PO Number 223-010, dated 30 October 2012, issued by Headquarters, 4th BCT, 82nd Airborne Division for the period 1 April 2009 to 9 December 2012

DISCUSSION AND CONCLUSIONS:

The applicant requests his award of an Army Commendation Medal with “V” Device be added to his DD Form 214.  A review of his record, however, shows not only this award, but an additional Army Commendation Medal.  His DD Form 214 should therefore be corrected to show the award of the Army Commendation Medal (3rd Award) as well as the Army Commendation Medal with “V” Device.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  deleting the entry on his DD Form 214 showing the award of the Army Commendation (2nd Award) and

	b.  adding to his DD Form 214 the Army Commendation Medal (4th Award) and with "V" Device.
