THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the: * Bronze Star Medal with "V" Device * Army Commendation Medal * Army Achievement Medal * Joint Service Achievement Medal 2. The applicant states the requested awards are missing. 3. The applicant provides: * DD Form 214 * Certificate showing award of the Bronze Star Medal * Certificate showing award of the Army Commendation Medal * Certificate showing award of the Army Achievement Medal * Certificate showing award of the Joint Service Achievement Medal CONSIDERATION OF EVIDENCE: 1. The applicant's records show he enlisted in the Regular Army on 10 June 2008 and he held military occupational specialty 11B (Infantryman). He served in Afghanistan from 24 August to 16 December 2009, 13 May to 18 October 2010, and 12 May to 2 September 2011. 2. He was honorably released from active duty on 20 October 2011. His DD Form 214 shows the: * Army Good Conduct Medal * National Defense Service Medal * Afghanistan Campaign Medal with two bronze service stars * Army Service Ribbon * Combat Infantryman Badge * Expert Infantryman Badge * Parachutist Badge 3. He provides three certificates confirming the requested awards by Permanent Orders (PO) as follows: * PO Number 91-03, issued by Headquarters, 1st Battalion, 75th Infantry Regiment (Ranger), on 1 April 2011, awarded him the Army Achievement Medal for meritorious achievement * PO Number 002-18, issued by Headquarters, 75th Ranger Regiment, on 2 January 2012, awarded him the Army Commendation Medal for meritorious service * PO Number 020-010, issued by Joint Task Force (Afghanistan) awarded him the Bronze Star Medal with "V" Device for heroism from 31 May to 1 June 2010 4. He also provides a certificate issued by Joint Task Force (Afghanistan) on 27 December 2010 awarding him the Joint Service Achievement Medal for meritorious service from 18 August to 16 December 2009. However, no accompanying permanent orders were provided. 5. Army Regulation 600-8-22 (Military Awards) states the Joint Service Achievement Medal was authorized by the Secretary of Defense on 3 August 1983. It is awarded to members of the Armed Forces of the United States below the grade of O-6 who, after 3 August 1983, distinguished themselves by meritorious achievement or service. DISCUSSION AND CONCLUSIONS: 1. PO awarded the applicant the Army Achievement Medal, Army Commendation Medal, and Bronze Star Medal with "V" Device, none of which is shown on his DD Form 214. Therefore, he is entitled to correction of his DD Form 214 to show all three awards. 2. As for award of the Joint Service Achievement Medal, there are no orders in his records and he provides none to confirm this award. The certificate he provides is insufficient without the orders to add it to his DD Form 214. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to his DD Form 214 the Army Achievement Medal, Army Commendation Medal, and Bronze Star Medal with "V" Device. 2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.
