THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of the Social Security Number (SSN) shown on his DD Form 214 (Report of Transfer or Discharge).
2. The applicant states the correct SSN is as shown on his Social Security card.
3. The applicant provides copies of his Social Security card and his DD Form 214.
CONSIDERATION OF EVIDENCE:
1. The applicant was inducted on 20 September 1966. He entered the SSN shown on his Social Security card on the DD Form 398 (Statement of Personal History) that he provided for his induction.
2. He was released from active duty (REFRAD) on 28 November 1968.
3. His REFRAD orders have the same SSN as his Social Security card, but the DD Form 214 has a different number.
DISCUSSION AND CONCLUSIONS:
The SSN on his DD Form 214 is wrong and should be corrected.
