THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her military records to show her full name as C____ A____-M____.
2. The applicant states her name was entered incorrectly in the military records system and no one has been able to correct it. When she married and changed her name, she hyphenated the new last name; however, when updating her name at that time, the military did not enter the hyphen or the records system did not recognize the symbol. She is not sure how it happened, but her last name appeared as A____, her first name as M____, and her middle name as C____. Her legal name is C____ A____-M____; she has no middle name (NMN).
3. The applicant provides her DD Form 214 (Certificate of Release or Discharge from Active Duty), her discharge orders, and her social security card.
CONSIDERATION OF EVIDENCE:
1. Having had prior active service in the U.S. Marine Corps under the last name A____, the applicant enlisted in U.S. Army Reserve (USAR) on 12 April 2000. Her enlistment contract shows she signed her name as C____ NMN A____-M____. She entered active duty on 23 September 2001.
2. Her service personnel records contain the following documents that show her name as C____ NMN A____-M____:
* DA Form 4836 (Oath of Extension of Enlistment or Reenlistment), dated 3 October 2001
* DA Form 4 (Enlistment/Reenlistment Document Armed Forces of the United States), dated 18 August 2004
* DD Form 2656 (Data for Payment of Retired Personnel), dated 22 May 2013
* her final DA Form 2166-8 (Noncommissioned Officer Evaluation), for the period 10 June 2012 to 9 June 2013
3. On 28 February 2014, she retired in the rank of sergeant first class.
4. Item 1 (Name (Last, First, Middle)) of her DD Form 214, for the period ending 28 February 2014, shows her her last name as A____, her first name as M____; and her middle name as C____.
5. Item 21a (Member Signature) of her DD Form 214 shows she electronically signed her name as A____-M____.C____.
6. She provided a copy of her social security card which shows her name as C____ A____-M____.
DISCUSSION AND CONCLUSIONS:
1. It appears an administrative error occurred during the preparation of the applicant's name on her DD Form 214 for the period ending 28 February 2014, since she electronically signed her name, in effect, as C____ A____-M____.
2. Her DD Form 2656 showing her name as C____ A____-M____ is accepted as sufficient evidence on which to base amending her military records to show this as her proper name.
