THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Afghanistan Campaign Medal and any other awards he is authorized for his service in Afghanistan.

2.  The applicant states the authorized awards were not added to his DD Form 214.

3.  The applicant provides an Army Commendation Medal Certificate and deployment orders.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 5 October 2010.

2.  Orders Number LS-285-0053, issued by Installation Management Command, Joint Base Lewis-McChord on 11 October 2012, ordered his deployment to Afghanistan in support of Operation Enduring Freedom (OEF), beginning on or about 8 November 2012.

3.  The Defense Finance and Accounting Service (DFAS) confirmed he received hostile fire pay/imminent danger pay (HF/IDP) and combat zone tax exclusion (CZTE) during the period 25 November 2012 - 25 April 2013 (5 months and 1 day).

4.  He was awarded the Army Commendation Medal for service in support of OEF.  

5.  He was honorably released from active duty on 7 February 2014.  His DD Form 214 shows in:

	a.  block 12f (Foreign Service) the entry "0000 00 00," indicating no foreign service was performed;

	b.  block 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) the:

* Army Commendation Medal
* Army Good Conduct Medal
* National Defense Service Medal
* Global War on Terrorism Service Medal
* Army Service Ribbon

	c.  block 18 (Remarks) does not reflect the dates of his service in Afghanistan.

6.  Army Regulation 600-8-22 (Military Awards) states the Afghanistan Campaign Medal is awarded to members who have served in direct support of OEF.  The area of eligibility encompasses all the land area of the country of Afghanistan and all air spaces above the land area.  The period of eligibility is on or after 
11 September 2001 to a future date to be determined by the Secretary of Defense or the cessation of OEF.  A bronze service star is authorized for wear with this medal for participation in each credited campaign.  Approved designated Afghanistan campaigns are:

* Liberation of Afghanistan (11 September 2001-30 November 2001)
* Consolidation I (1 December 2001-30 September 2006)
* Consolidation II (1 October 2006-30 November 2009)
* Consolidation III (1 December 2009 – 30 June 2011)
* Transition I (1 July 2011-date to be determined)

7.  Army Regulation 635-5 (Separations Documents), in effect during the applicant's active duty service, prescribed the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It established standardized policy for the preparation of the DD Form 214.  It stated the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active service.  It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge.  The regulation provided that for:

	a.  block 12f, enter the total amount of foreign service completed during the period covered by the DD Form 214;

	b.  block 13, list awards and decorations for all periods of service in the priority sequence specified in Army Regulation 600-8-22; and

	c.  block 18, for an active duty Soldier deployed to a foreign country with his or her unit during their continuous period of active service, enter the statement "SERVICE IN (name of country deployed) FROM (inclusive dates, for example, YYYYMMDD-YYYYMMDD)."

DISCUSSION AND CONCLUSIONS:

1.  The applicant's request for award of the Afghanistan Campaign Medal has been carefully considered.  

2.  Orders show he was ordered to deploy to Afghanistan in support of OEF with a departure date of on or about 8 November 2012.  DFAS records confirm he received HF/IDP and CZTE during the period 25 November 2012 - 25 April 2013 (5 months and 1 day).  Therefore, his DD Form 214 should be corrected to reflect the amount of foreign service performed in block 12f and to document his deployment to Afghanistan in block 18.

3.  Based on his service in Afghanistan, he is authorized the Afghanistan Campaign Medal with one bronze service star and correction of his DD Form 214 to show this award.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  deleting the current entry in block 12f of his DD Form 214 and adding the entry "0000 05 01;

	b.  adding to his DD Form 214 the Afghanistan Campaign Medal with one bronze service star; and

	c.  adding to block 18 of his DD Form 214 the entry "SERVICE IN AFGHANISTAN FROM 20121125-20130425."
