THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show his rank as Private First Class (PFC) E-3 and his deployment to Iraq.

2.  The applicant states that his DD Form 214 does not reflect his correct rank or his deployment dates.

3.  The applicant provides of his deployment and separation orders.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 11 May 2010.  He completed his training as a motor transport operator and was assigned to Fort Stewart, GA.

2.  He deployed to Iraq during the period 20110304 - 20110618 (3 months and 15 days of foreign service) and was advanced to the pay grade of E-3 on 1 April 2011.

3.  On 31 December 2012, he was honorably discharged under the provisions of Army Regulation 635-200, paragraph 13-2e, due to physical standards.  He had served 2 years, 7 months, and 20 days of active service and his DD Form 214 issued at the time of his discharge shows his rank as “PV2 E02.”  It also shows no foreign service in item 12f and no deployment dates in item 18 (Remarks).

4.  Army Regulation 635-5 at the time served as the authority for the preparation of the DD Form 214.  It provides, in pertinent part, that for an active duty Soldier deployed with his or her unit during their continuous period of active service, the following entry will be made in item 18:  "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)."

DISCUSSION AND CONCLUSIONS:

1.  The applicant was advanced to the rank and pay grade of PFC E-3 on 1 April 2011 and is entitled to have that advancement reflected on his DD Form 214.

2.  The applicant also served 3 months and 15 days of foreign service in Iraq during the period 20110304 – 20110618 and is entitled to have that information reflected on his DD Form 214 at this time.

BOARD DETERMINATION/RECOMMENDATION:

1. The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

* Deleting the entry “PV2” from item 4a of his DD Form 214
* Deleting the entry “E02” from item 4b of his DD Form 214
* Deleting the entry “0000 00 00” from item 12f of his DD Form 214
* Deleting the entry “2010 05 11” from item 12i of his DD Form 214
* Adding the entry “PFC” to item 4a of his DD Form 214
* Adding the entry “E03” to item 4b of his DD Form 214
* Adding the entry “0000 03 15” to item 12f of his DD Form 214
* Adding the entry “2011 04 01” to item 12i of his DD Form 214
* Adding the entry “Service in Iraq 20110304 – 20110618” to item 18 of his DD Form 214

2.  The Board wants the applicant and all others to know that the sacrifices he made in service to the United States during the Global War on Terrorism are deeply appreciated.  The applicant and all Americans should be justifiably proud of his service in arms.
