THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show he was awarded the Army Commendation Medal (2nd Award) and the Meritorious Unit Commendation (MUC).

2.  The applicant states he received the awards after his discharge. 

3.  The applicant provides copies of the Army Commendation Medal (2nd Award) and the MUC orders.

CONSIDERATION OF EVIDENCE:

1.  The applicant was commissioned as a Regular Army captain in the Medical Service Corps and entered active duty on 18 September 2007 as a clinical psychologist intern.

2.  He was stationed at Fort Hood, Texas when he deployed to Iraq with the 833rd Medical Company during the period 20100723 – 20110727.

3.  On 18 October 2011, he was honorably discharged due to miscellaneous/general reasons, after completing 4 years, 1 month, and 1 day of active service.  His DD Form 214 shows he was awarded the Iraq Campaign Medal with two campaign stars, the Army Commendation Medal, the National Defense Service Medal, the Global War on Terrorism Service Medal, the Army Service Ribbon, the Overseas Service Ribbon, and the Combat Action Badge.
4.  His records show he was awarded the following awards:

* Army Commendation Medal, in Permanent Orders Number 161-001, issued by Headquarters, Medical Task Force 86 in Baghdad, Iraq for the period 30 July 2010 – 21 July 2011
* Army Commendation Medal (2nd Award), in Permanent Orders Number 262-03, issued by Headquarters, Carl R. Darnall Army Medical Center, Fort Hood, Texas for the period 9 April 2010 – 21 September 2011
* Army Achievement Medal, in Permanent Orders Number 197-01, issued by Headquarters, Fort Campbell, Kentucky for the period 14 June 2009 – 11 September 2009
* Meritorious Unit Commendation, issued by the U.S. Army Human Resources Command, to the 883rd Medical Company for the period 
19 September 2010 – 23 July 2011 

 DISCUSSION AND CONCLUSIONS:

The applicant was awarded the Army Commendation Medal (2nd Award), the Army Achievement Medal, and his unit was awarded the MUC, which are not reflected on his DD Form 214.  Accordingly, his DD Form 214 should be corrected to show those awards.

BOARD DETERMINATION/RECOMMENDATION:

1. The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

* amending his DD Form 214 to delete the Army Commendation Medal
* amending his DD Form 214 to add the Army Commendation Medal (2nd Award), the Army Achievement Medal, and the MUC

2.  The Board wants the applicant and all others to know that the sacrifices he made in service to the United States during the Global War on Terrorism are deeply appreciated.  The applicant and all Americans should be justifiably proud of his service in arms.
