THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of item 12f (Foreign Service) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his foreign service in Iraq.

2.  The applicant states he served in Iraq twice:  29 April 2007 to 12 July 2008 and 28 October 2009 to 24 October 2010.  Item 12f of his DD Form 214 does not show his foreign service. 

3.  The applicant provides copies of his Enlisted Record Brief (ERB), dated 
24 July 2013, and his DD Form 214.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 28 February 2006.  He held military occupational specialty 13F (Fire Support Specialist).

2.  His ERB shows he served in Iraq from 29 April 2007 to 12 July 2008 (1 year, 2 months, and 14 days) and from 28 October 2009 to 24 October 2010 
(11 months and 27 days).  (He served in Iraq for a total of 2 years, 2 months and 11 days).

3.  He was retired due to permanent disability effective 27 September 2013 as a specialist/E-4.  He had completed 7 years and 7 months of active duty service.  His DD Form 214 shows in:

	a.  item 12f (Foreign Service):  "0000 00 00" (indicating no foreign service);

	b.  item 18 (Remarks):  in part, service in Iraq from 29 April 2007 to 
12 July 2008 and from 28 October 2009 to 24 October 2010.

4.  An email from the Defense Finance and Accounting Service, dated 
26 February 2014, confirmed he received hostile fire pay (HFP)/imminent danger pay (IDP) for service in Kuwait from 29 April 2007 through 9 July 2008 and 
29 October 2009 through 24 October 2010.

5.  Army Regulation 635-8 (Separation Processing and Documents) establishes the standardized policy for preparing and distributing the DD Form 214.  It is important that information entered on the form should be complete and accurate. 
The DD Form 214 is a summary of a Soldier's most recent period of continuous active duty.  Item 12f shows the total amount of foreign service completed during the period. 

DISCUSSION AND CONCLUSIONS:

Although DFAS' confirmation indicates he was deployed only to Kuwait, his record shows he also served in Iraq.  He served in Kuwait/Iraq from 29 April 2007 to 12 July 2008 and from 28 October 2009 to 24 October 2010.  Item 12f of his DD Form 214 should be corrected to show his foreign service as "0002 02 11."

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by  deleting the current entry in item 12f of his DD Form 214 and replacing it with
"02 02 11."
