THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant, the widow of a deceased former service member (FSM), requests correction of her husband’s social security number (SSN) on his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show XXX-XX-XX24 instead of XXX-XX-XX25.

2.  The applicant states:

* the FSM did not have a chance to change his SSN because he was in poor health
* please correct the FSM's SSN on all of his military records 

3.  The applicant provides the FSM's:

* DD Form 214
* Social Security card
* two orders

CONSIDERATION OF EVIDENCE:

1.  The FSM enlisted in the U.S. Army Reserve (USAR) and was ordered to active duty for training (ADT) on 5 July 1962.  Item 37 (Remarks) of his DD Form 4 (Enlisted Record - Armed Forces of the United States) shows his SSN as XXX-XX-XX24.

2.  The applicant's record contains Letter Orders Number 2732, issued by Headquarters, XII U.S. Army Corps, Atlanta, GA, dated 18 June 1962, which show his SSN as XXX-XX-XX24.

3.  On 14 December 1962, the FSM was released from ADT.  Item 32 (Remarks) of the FSM's DD Form 214 shows his SSN as XXX-XX-XX25.

4.  The following documents contained in the FSM's record and that were submitted by the applicant show his SSN as XXX-XX-XX24: 

	a.  Special Orders Number 36, issued by Headquarters, U.S. Army Advisor Group (U.S. Army Reserve) Hawaii, dated 21 March 1968; and 

	b.  Special Orders Number 64, issued by Headquarters, U.S. Army Advisor Group (U.S. Army Reserve) Hawaii, dated 14 May 1968.

5.  The applicant submitted the FSM's social security card that shows the FSM's SSN as XXX-XX-XX24.

6.  Army Regulation 635-5 (Separation Documents) then in effect governed the preparation of the DD Form 214.  The regulation provided that the Soldier's SSN would be entered in item 6 of the DD Form 214.

DISCUSSION AND CONCLUSIONS:

The documents in the FSM's record show two different social security numbers when he served in an active status including the one the applicant is requesting.  The FSM's social security card also matches the SSN under which he enlisted and served.  It appears a typographical error was made.  Therefore, his DD Form 214 should be corrected to show his SSN as shown on his social security card.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting the current entry in item 32 of the FSM's DD Form 214 and adding the SSN as it is listed on the FSM's social security card.
