THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Meritorious Unit Commendation.
2. The applicant states this award was approved after she signed her DD Form 214 and she would like it added to her record.
3. The applicant provides her DD Form 214 and Permanent Orders (PO) Number 021-16, dated 21 January 2011.
CONSIDERATION OF EVIDENCE:
1. On 2 February 2007, the applicant enlisted in the U.S. Army Reserve as an officer candidate. On 15 February 2007, she entered active duty for the purpose of attending Officer Candidate School, and on 1 August 2007, she was released from active duty for the purpose of accepting a commission in the Army.
2. Although her oath of office is not in her record, she appears to have been appointed as a commissioned officer on 2 August 2007. She held the area of concentration 67A (Health Services) and attained the rank of captain.
3. Her Officer Record Brief, Officer Evaluation Report for the rating period
31 July 2009 to 10 July 2010, and a Certificate of Appreciation shows she was assigned to the 51st Medical Logistics Company, 118th Multifunctional Medical Battalion.
4. She provided and her record contains PO Number 021-16, issued by Headquarters, U.S. Army Human Resources Command on 21 July 2011. These orders show the 51st Medical Company was awarded the Meritorious Unit Commendation for the period 21 October 2009 to 18 July 2010 for exceptional meritorious performance in support of Operation Iraqi Freedom.
5. She was honorably discharged on 30 September 2011. Her DD Form 214 shows she deployed to Iraq in support of Operation Iraqi Freedom from
5 November 2009 to 23 July 2010. Her DD Form 214 shows she was last assigned to the 51st Medical Logistics Company.
DISCUSSION AND CONCLUSIONS:
Orders awarded her unit the Meritorious Unit Commendation during the period in which she was assigned; therefore, she is entitled to correction of her DD Form 214 to show this unit award.
