THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests removal of the following documents from his official military personnel file (OMPF):
* duplicate award of a foreign badge, dated 26 January 2001, that was filed in his OMPF on 13 November 2013
* a blank document, titled "Course Completion," that was filed in his OMPF on 3 March 2013
2. The applicant states his OMPF contains a duplicate document and a blank document that should be removed.
a. The first document pertains to an award of the Tunisian Parachutist Badge. A memorandum, issued by Headquarters, U.S. Army Southern Command European Task Force (Airborne), dated 26 January 2001, awarded him this badge. The award approval memorandum contains his name and social security number (SSN) and other individuals' names and SSNs. However, the other individuals' SSNs are darkened. This memorandum was filed correctly in his OMPF on 6 November 2013. The same memorandum, a duplicate, with other individuals' visible SSNs was also filed in his OMPF on 13 November 2013. The duplicate document should be removed.
b. A blank document titled "Course Completion" was filed in his OMPF on 3 March 2007. He does not know why and it is annoying to him. His records will be reviewed by a promotion board and he wants this document removed.
3. The applicant does not provide any evidence.
CONSIDERATION OF EVIDENCE:
1. Having had prior service, the applicant enlisted in the U.S. Army Reserve on 7 September 2006. He is currently serving on active duty in the Active Guard Reserve program. His OMPF contains the following documents:
a. A memorandum issued by Headquarters, U.S. Army Southern Command European Task Force (Airborne), dated 26 January 2001, awarding him the Tunisian Parachutist Badge. This award approval memorandum contains his name and other individuals' names and SSNs; however, the other individuals' SSNs are lined through/darkened. This memorandum was filed in his OMPF on 6 November 2013.
b. A duplicate memorandum, also issued by Headquarters, U.S. Army Southern Command European Task Force (Airborne), dated 26 January 2001, awarding him the same badge. This memorandum contains his name and SSN as well as other individuals' names and visible SSNs. This memorandum was filed in his OMPF on 13 November 2013.
c. A blank document titled "Course Completion" was filed in his OMPF on 3 March 2007.
2. Army Regulation 600-8-104 (Army Military Human Resource Record Management) prescribes the policies and mandated operating tasks for the Military Personnel Information Management/Records Program of the Military Personnel System. The regulation provides detailed guidance and instructions regarding the initiation, composition, maintenance, changing, access to, and transfer of the OMPF. Table B-2 shows that award orders (including badges and tabs) are filed in the performance folder of the OMPF.
DISCUSSION AND CONCLUSIONS:
1. The purpose of maintaining the OMPF is to protect the interests of both the Army and the Soldier. In this regard, the OMPF serves to maintain an unbroken, historical record of a Soldier's service, conduct, duty performance, and evaluation periods and any corrections to other parts of the OMPF. Once placed in the OMPF, the document becomes a permanent part of that file and will not be removed from or moved to another part of the OMPF unless directed by an appropriate authority.
2. The evidence of record shows the applicant's OMPF contains duplicate memoranda authorizing him and other individuals an award of a foreign badge. One memorandum, filed on 6 November 2013, contains lined through/darkened SSNs of all those listed accept the applicant's while the duplicate document, filed on 13 November 2013, has the names and SSNs of other individuals visible. The existence of the duplicate document is a detractor that stands out as soon as his records are reviewed and may give the appearance that he is neglecting to review his OMPF. The memorandum with the visible SSNs should be removed from his OMPF.
3. The evidence of record also shows the applicant's OMPF contains a blank document that was added to his OMPF on 3 March 2007. This document serves no purpose and its presence is also a detractor. The memorandum should be removed from his OMPF

a. removing from his OMPF the memorandum, issued by Headquarters, U.S. Army Southern Command European Task Force (Airborne), dated 26 January 2001, awarding him the Tunisian Badge that contains his name and other individual's names and visible social security numbers. This memorandum was filed on 13 November 2013; and
b. removing from his OMPF a blank document titled "Course Completion," which was filed in his OMPF on 3 March 2007.
