THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his service in Afghanistan and all authorized awards.
2. The applicant states his deployment to Afghanistan is not shown on his DD Form 214. He has post-traumatic stress disorder (PTSD) and can apply for benefits if his DD Form 214 is corrected.
3. The applicant provides a copy of his travel voucher for the deployment to Afghanistan.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army in 17 August 1999. He held military occupational specialty 92M (Mortuary Affairs Specialist).
2. He was honorably released from active duty on 20 December 2003 as a specialist (SPC)/E-4. He had completed 4 years, 4 months, and 4 days of net active duty service. His DD Form 214 shows in:
a. Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized), the Army Lapel Button, Drivers Badge, National Defense Service Medal, and Army Service Ribbon.
b. item 18 (Remarks), shows "SERVICE IN SOUTHWEST ASIA 20020413-20021203."
3. The Defense Finance and Accounting Service (DFAS) verified that the applicant's Master Military Pay Account (MMPA) shows he received hostile fire/imminent danger pay in Afghanistan from 30 April through 3 December 2002.
4. Permanent Orders 158-14, 377th Theater Support Command, shows he was awarded the Army Commendation Medal on 5 June 2003 for meritorious service during Operation Enduring Freedom during the period 5 May to 14 October 2002.
5. Army Regulation 600-8-22 (Military Awards) states the:
a. Army Good Conduct Medal is awarded to individuals who distinguish themselves by their conduct, efficiency, and fidelity. Although there is no automatic entitlement to the Army Good Conduct Medal, disqualification must be justified. The following period of continuous enlisted active Federal military service qualifies for award: For first award only, upon termination of service on or after 27 June 1950 of less than 3 years but more than 1 year.
b. Global War on Terrorism Service Medal (GWOTSM) is authorized for award to all Soldiers on active duty on or after 11 September 2001 to a date to be determined.
c. Afghanistan Campaign Medal is awarded to members who have served in direct support of Operation Enduring Freedom (OEF). The area of eligibility encompasses all the land area of the country of Afghanistan and all air spaces above the land area. The period of eligibility is on or after 11 September 2001 to a future date to be determined by the Secretary of Defense or the cessation of OEF. A bronze service star is authorized for wear with this medal for participation in each credited campaign. During his service in Afghanistan he participated in the Consolidation I (1 December 2001-30 September 2006)
campaign.
DISCUSSION AND CONCLUSIONS:
1. The applicants records show he distinguished himself in the performance of his military service as evidenced by him attaining the rank of SPC and receiving a personal decoration. Furthermore, there is no evidence of any misconduct or disciplinary action or a commander's disqualification for award of the Army Good Conduct Medal. It is presumed that he did not receive the Army Good Conduct Medal due to an oversight. Accordingly, he should be awarded the Army Good Conduct Medal (1st Award).
2. The available evidence shows he served qualifying periods for award of the Global War on Terrorism Service Medal and Afghanistan Campaign Medal with one bronze service star.
3. Permanent orders show he was awarded the Army Commendation Medal.
4. His DD Form 214 should be corrected to show these awards.
5. His DD Form 214 should be corrected to show his deployment to Afghanistan.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined the evidence presented is sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. awarding him the Army Good Conduct Medal (1st Award) for the period
17 August 1999 through 16 August 2002;
b. adding to item 13 of his DD Form 214 the:
* Army Good Conduct Medal (1st Award)
* Army Commendation Medal
* Global War on Terrorism Service Medal
* Afghanistan Campaign Medal with one bronze service star
c. deleting the entry in item 18 stating, "SERVICE IN SOUTHWEST ASIA 20020413-20021203" and replacing it with the entry, "SERVICE IN AFGHANISTAN 20020430-20021203."
