THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Combat Infantryman Badge and Meritorious Unit Commendation. 

2.  The applicant states these awards were omitted from his DD Form 214.  He did not notice this until a few months later when he was going through his personal military records.

3.  The applicant provides copies of his Officer Record Brief, Permanent Orders (PO) Number 098-012 and 069-34, and DD Form 214.

CONSIDERATION OF EVIDENCE:

1.  The applicant was commissioned in the Regular Army on 26 May 2007 and held area of concentration 11A (infantry).  

2.  He provided copies of the following:

* PO Number 098-012, issued by Headquarters, 1st Battalion, 25th Stryker Brigade Combat Team, dated 8 April 2009, awarding him the Combat Infantryman Badge while serving with the 1st Battalion, 5th Infantry Regiment
* PO Number 069-34, issued by the U.S. Army Human Resources Command, dated 10 March 2010, awarding members of the 1st Battalion, 5th Infantry Regiment, the Meritorious Unit Commendation, for the period 28 October 2008 through 12 September 2009

3.  He was honorably released from active duty in the rank of captain on 30 August 2012.  He completed 5 years, 3 months, and 5 days of net active service.  His DD Form 214 does not list the Combat Infantryman Badge and Meritorious Unit Commendation.

DISCUSSION AND CONCLUSIONS:

PO awarded the applicant the Combat Infantryman Badge and his unit the Meritorious Unit Commendation during his period of active duty.  Therefore, he is entitled to correction of his DD Form 214 to show these awards.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to his DD Form 214 the Combat Infantryman Badge and Meritorious Unit Commendation.
