THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of item 15b (Commissioned Through Reserve Officers' Training Corps (ROTC) Scholarship) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to read "Yes."

2.  The applicant states, in effect, the entry in item 15b is incorrect.  He was awarded a 2-year Army ROTC Scholarship on 20 August 2010.

3.  The applicant provides:

* ROTC Cadet Command Form 167-R (U.S. Army ROTC Scholarship Acceptance/Declination Statement and Professor of Military Science Validation)
* DD Form 4 (Enlistment/Reenlistment Document – Armed Forces of the United States)
* DD Form 214

CONSIDERATION OF EVIDENCE:

1.  The applicant provided his ROTC Cadet Command Form 167-R, dated 20 August 2010, which shows he accepted a 2-year campus-based ROTC Scholarship.

2.  He also provided his DD Form 4 which shows he enlisted in the U.S. Army Reserve (USAR) Senior ROTC Program as a cadet on 20 August 2010.

3.  He was appointed as a second lieutenant in the USAR on 5 May 2012 and entered on active duty for training (ADT) on 16 October 2012.  He was released from ADT on 9 March 2013.  Item 15b of his DD Form 214 shows the block was checked "No."

4.  He is currently serving in an active Reserve status.

DISCUSSION AND CONCLUSIONS:

The applicant's contention that item 15b of his DD Form 214 is incorrect was carefully reviewed and found to have merit.  The evidence of record confirms he was commissioned in the USAR through the Senior ROTC Scholarship Program on 20 August 2010.  Therefore, item 15b of his DD Form 214 should be corrected to show an "X" in the "Yes" block.
