THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show three awards of the Army Achievement Medal.

2.  The applicant states his DD Form 214 shows he has two Army Achievement Medals but he has received three.  His DD Form 214 should be corrected.

3.  The applicant provides copies of three DA Forms 638 (Recommendation for Award) showing three awards of the Army Achievement Medal and his DD Form 214.

CONSIDERATION OF EVIDENCE:

1.  The applicant's records show he enlisted in the Regular Army on 16 June 2009 and he held military occupational specialty 91B (Wheeled Vehicle Mechanic). 

2.  He was honorably released from active duty on 30 November 2012 as a specialist (SPC)/E-4.  He had completed 3 years, 5 months, and 15 days of active duty service.  His DD Form 214 shows he was awarded or authorized the

* Army Achievement Medal (2nd Award)
* Army Good Conduct Medal
* National Defense Service Medal
* Global War on Terrorism Service Medal
* Korea Defense Service Medal
* Army Service Ribbon
* Overseas Service Ribbon

3.  In support of his request, the applicant provides copies of the DA Forms 638 showing he was awarded the Army Achievement Medal by:

	a.  Permanent Order Number 348-04, issued by Division Special Troops Battalion (DSTB), on 28 December 2010, for service from 21 January 2010 to 
19 January 2011.

	b.  Permanent Order Number 208-13, issued by Headquarters, 1st Battalion, 35th Armor, 2d Heavy Brigade Combat Team, 1st Armored Division, Fort Hood, Texas, on 27 July 2011, for achievement from 6 June 2011 to 13 July 2011. 

	c.  Permanent Order Number 285-01, issued by Headquarters, 1st Battalion, 35th Armor, 2d Heavy Brigade Combat Team, 1st Armored Division, Fort Hood, Texas, on 11 October 2012, for service from 8 February 2009 to 14 October 2012.  

DISCUSSION AND CONCLUSIONS:

The applicant was awarded three awards of the Army Achievement Medal.  Only two are listed on his DD Form 214.  He is entitled to correction of his 
DD Form 214 to show the third award.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 as follows:

	a.  deleting the Army Achievement Medal (2nd Award) and

	b.  adding the Army Achievement Medal (3rd Award).
