THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Forms 214 (Certificate of Release or Discharge from Active Duty) to show the proper dates and time served.
2. The applicant states her DD Forms 214 have overlapping periods and her active duty service and prior service are not properly reflected.
3. The applicant provides copies of her DD Forms 214, a letter from the Illinois Student Assistance Commission, Chronological Statement of Retirement Points, and mobilization orders with amendments.
CONSIDERATION OF EVIDENCE:
1. The applicant was serving in the U.S. Army Reserve (USAR) in the rank of captain when she was ordered to active duty on 1 March 2009 in support of Operation Enduring Freedom. She was honorably released from active duty (REFRAD) on 28 February 2011. She completed 1 year, 11 months, and 28 days of net active service during this period.
2. Her DD Form 214 issued at the time of her REFRAD shows she completed 2 years of net active service during this period and she had no prior active service.
3. On 1 March 2011, she was again ordered to active duty and she was honorably REFRAD on 6 January 2012. She completed 10 months and 6 days of net active service during this period. Item 12d (Total Prior Active Service) of her DD Form 214 shows she had 2 years, 5 months, and 26 days of prior active service.
4. On 7 January 2012, she was again ordered to active duty and she was honorably REFRAD on 12 September 2012. She competed 8 months and 6 days of net active service during this period. However, item 12a (Date Entered Active Duty This Period) of her DD Form 214 issued at the time of her REFRAD reflects that she entered active duty on 1 March 2009 and item 12d shows she had 3 years, 4 months, and 2 days of prior active service.
DISCUSSION AND CONCLUSIONS:
1. The applicant completed 2 years of prior active service according to her DD Form 214 for the period ending 28 February 2011. However, when she was REFRAD on 6 January 2012, her DD Form 214 reflects that she had 2 years, 5 months, and 26 days of prior active service. Accordingly, her DD Form 214 for the period ending 6 January 2012 should be corrected to show she had 2 years of prior active service in item 12d.
2. The applicant was again ordered to active duty on 7 January 2012 and she was REFRAD on 12 September 2012 for a period of 8 months and 6 days of net active service. However, her DD Form 214 for the period ending 12 September 2012 incorrectly reflects that she entered active duty on 1 March 2009 and that she had 3 years, 4 months, and 2 days of prior active service. Accordingly, her DD Form 214 for the period ending 12 September 2012 should be corrected to show she entered active duty on 7 January 2012 and she had 4 years, 5 months, and 26 days of prior active service.

* deleting the entry "0003 04 02" from item 12d of her DD Form 214 for the period ending 12 September 2012
