BOARD DATE: 11 June 2013
DOCKET NUMBER: AR20120021278
THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show his social security number (SSN) as "x3x-xx-xxxx" instead of "x2x-xx-xxxx."
2. The applicant states it has been an oversight all these years.
3. The applicant provides:
* Social Security card
* DD Form 214
CONSIDERATION OF EVIDENCE:
1. Title 10, U.S. Code, section 1552(b), provides that applications for correction of military records must be filed within 3 years after discovery of the alleged error or injustice. This provision of law also allows the Army Board for Correction of Military Records (ABCMR) to excuse an applicant's failure to timely file within the 3-year statute of limitations if the ABCMR determines it would be in the interest of justice to do so. While it appears the applicant did not file within the time frame provided in the statute of limitations, the ABCMR has elected to conduct a substantive review of this case and, only to the extent relief, if any, is granted, has determined it is in the interest of justice to excuse the applicant's failure to timely file. In all other respects, there are insufficient bases to waive the statute of limitations for timely filing.
2. The applicant was inducted into the Army of the United States on 19 February 1964.
3. His DA Form 41 (Record of Emergency Data), dated 1965, shows his SSN as "x3x-xx-xxxx."
4. He served as a supply handler in Okinawa from October 1965 to February 1966. On 10 February 1966, he was honorably released from active duty as an overseas returnee and he was transferred to the U.S. Army Reserve (USAR) Control Group (Annual Training) to complete his remaining service obligation. He completed 1 year, 1 month, and 22 days of total active service. The DD Form 214 he was issued at the time shows in item 32 (Remarks) his SSN as
"x2x-xx-xxxx."
5. His record contains a DA Form 20 (Enlisted Qualification Record) that shows his SSN as "x3x-xx-xxxx."
6. He provides a copy of his Social Security card that shows his SSN as
"x3x-xx-xxx."
DISCUSSION AND CONCLUSIONS:
The applicant's Social Security card, DA Form 41, and DA Form 20 shows his SSN as "x3x-xx-xxxx." It appears that during the preparation of his DD Form 214 an administrative error occurred that resulted in the second digit of his SSN being incorrectly reflected. Therefore, it would be appropriate at this time to correct his DD Form 214 to show his SSN as shown on his Social Security card, DA Form 41, and DA Form 20.
BOARD VOTE:

IN THE CASE OF: BOARD DATE: 3 April 2012 DOCKET NUMBER: AR20110019056 THE BOARD CONSIDERED THE FOLLOWING EVIDENCE: 1. He completed a DD Form 398 (Statement of Personal History) in connection with his enlistment and shows his SSN as "x3x-xx-xxxx." Item 3 (SSN) of the DD Form 214 he was issued at the time shows his SSN as "x2x-xx-xxxx."
ARMY | BCMR | CY2013 | 20130017605
The applicant's military record contains a DD Form 398 (Statement of Personal History) which was completed during his enlistment processing. His record clearly lists this SSN on all applicable documents maintained throughout his military service. Absent convincing independent and verifiable evidence to the contrary, it is presumed that the applicant's military service records, including the DD Form 214, were correct at the time and there is insufficient evidence to grant relief in this case.
ARMY | BCMR | CY2014 | 20140008192
The applicant requests correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show he is authorized two Meritorious Unit Commendations (MUC) and to show his social security number (SSN) as "X8X-XX-XXXX" vice "X3X-XX-XXXX." The evidence of record shows he participated in two campaigns while serving in Vietnam; therefore, he is entitled to correction of his DD Form 214 to show two bronze service stars to be affixed to his previously-awarded...

The applicant requests correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) and other military records to show his social security number (SSN) as "X1X-XX-XXXX" instead of "X3X-XX-XXXX." However, his DA Form 20 and his DD Form 214 were changed by pencil to show the same SSN shown on his DD Form 398 and his DA Form 41. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending...

Application for correction of military records (with supporting documents provided, if any). The applicant requests correction of item 3 (Social Security Number (SSN)) of her DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show the number "2" instead of the number "3" in the second digit. The evidence of records shows that the SSN that contained the number "2" in the second digit is consistent with the SSN used throughout her military service.
ARMY | BCMR | CY2013 | 20130005795
The applicant states the SSN shown on his DD Form 214 is incorrect. The applicant requests correction of item 3 (Social Security Number (SSN)) of his DD Form 214 (Report of Separation from Active Duty) to show "2" as the second digit instead of "3."
