IN THE CASE OF:
BOARD DATE: 26 November 2014
DOCKET NUMBER: AR20130005795
THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show his social security number (SSN) as "x2x-xx-xxxx."
2. The applicant states the SSN shown on his DD Form 214 is incorrect. He states the incorrect SSN is a problem with the Department of Veterans Affairs (VA) processing his claim paperwork.
3. The applicant provides his DD Form 214 and a photocopy of his Social Security card.
CONSIDERATION OF EVIDENCE:
1. Title 10, U.S. Code, section 1552(b), provides that applications for correction of military records must be filed within 3 years after discovery of the alleged error or injustice. This provision of law also allows the Army Board for Correction of Military Records (ABCMR) to excuse an applicant's failure to timely file within the 3-year statute of limitations if the ABCMR determines it would be in the interest of justice to do so. While it appears the applicant did not file within the time frame provided in the statute of limitations, the ABCMR has elected to conduct a substantive review of this case and, only to the extent relief, if any, is granted, has determined it is in the interest of justice to excuse the applicant's failure to timely file. In all other respects, there are insufficient bases to waive the statute of limitations for timely filing.
2. On 15 April 1965, the applicant was inducted into the Army of the United States.
3. His DD Form 47 (Record of Induction) shows his SSN as "x2x-xx-xxxx."
4. On 14 April 1967, he was honorably released from active duty at the expiration of his term of service. His DD Form 214 shows his SSN as
"x4x-xx-xxxx."
5. The applicant provides his Social Security card that shows his SSN as
"x2x-xx-xxxx."
DISCUSSION AND CONCLUSIONS:
1. The applicant contends his SSN is incorrect on his DD Form 214.
2. The applicant's DD Form 214 shows his SSN as "x4x-xx-xxxx"; however, his DD Form 47 and his Social Security card show his SSN as "x2x-xx-xxxx." It appears that during the preparation of his DD Form 214 an administrative error occurred resulting in the wrong SSN being entered on his DD Form 214.
3
. In view of the foregoing, it would be appropriate at this time to correct his
DD Form 214 to show his SSN as shown on his Social Security card.
BOARD VOTE:

BOARD DETERMINATION/RECOMMENDATION:
The Board determined the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by
deleting from his DD Form 214 the current SSN entry and replacing it with the SSN as shown on his Social Security card and DD Form 47.

The applicant requests correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to correct his social security number (SSN). His DD Form 214 shows in item 3 (Social Security Number) his SSN as X2X-XX-XXXX. However, evidence of record shows he submitted a USAAC Form 170 which shows he requested that his record be corrected to show his SSN as X4X-XX-XXXX while still serving in the Reserves.
ARMY | BCMR | CY2013 | 20130003088
The applicant requests correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show his social security number (SSN) as "X4X-XX-XXXX." His DD Form 47 (Record of Induction) and other documents throughout his military record show his SSN as "X4X-XX-XXXX." However, the other documents in his military record containing his SSN and his social security card show his SSN as "X4X-XX-XXXX."
ARMY | BCMR | CY2012 | 20120012566
IN THE CASE OF: BOARD DATE: 10 January 2013 DOCKET NUMBER: AR20120012566 THE BOARD CONSIDERED THE FOLLOWING EVIDENCE: 1. There are no documents in his available record, other than the DD Form 214, that show his SSN as x2x-xx-xxxx during his active duty service. DISCUSSION AND CONCLUSIONS: The applicant's record contains several documents that show he used the same SSN as it appears on his Social Security card during his active duty service.
ARMY | BCMR | CY2010 | 20100017319
The applicant requests correction of item 3 (Social Security Number (SSN)) of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show "X9X-XX-XXXX" instead of "X4X-XX-XXXX." His DD Form 398 (Statement of Personal History), dated 6 December 1965, shows his SSN as "X9X-XX-XXXX." _______ _X _______ ___ CHAIRPERSON I certify that herein is recorded the true and complete record of the proceedings of the Army Board for Correction of Military Records in this case.
ARMY | BCMR | CY2012 | 20120004816
IN THE CASE OF: BOARD DATE: 6 September 2012 DOCKET NUMBER: AR20120004816 THE BOARD CONSIDERED THE FOLLOWING EVIDENCE: 1. His DD Form 47 (Record of Induction) does not list an SSN. Therefore, it would be appropriate at this time to correct his DD Form 214 accordingly.
ARMY | BCMR | CY2011 | 20110019056
IN THE CASE OF: BOARD DATE: 3 April 2012 DOCKET NUMBER: AR20110019056 THE BOARD CONSIDERED THE FOLLOWING EVIDENCE: 1. He completed a DD Form 398 (Statement of Personal History) in connection with his enlistment and shows his SSN as "x3x-xx-xxxx." Item 3 (SSN) of the DD Form 214 he was issued at the time shows his SSN as "x2x-xx-xxxx."
ARMY | BCMR | CY2014 | 20140007047
Application for correction of military records (with supporting documents provided, if any). The applicant provides his DD Form 214. The evidence of record shows his HOR address was listed as 726 Park Avenue throughout his military service.
ARMY | BCMR | CY2012 | 20120004014
IN THE CASE OF: BOARD DATE: 16 August 2012 DOCKET NUMBER: AR20120004014 THE BOARD CONSIDERED THE FOLLOWING EVIDENCE: 1. Application for correction of military records (with supporting documents provided, if any). The evidence of record confirms the applicant provided his correct SSN at the time of his induction, as evidenced by the DD Form 398 and DA Form 3027 in the record.
ARMY | BCMR | CY2013 | 20130017605
The applicant's military record contains a DD Form 398 (Statement of Personal History) which was completed during his enlistment processing. His record clearly lists this SSN on all applicable documents maintained throughout his military service. Absent convincing independent and verifiable evidence to the contrary, it is presumed that the applicant's military service records, including the DD Form 214, were correct at the time and there is insufficient evidence to grant relief in this case.
ARMY | BCMR | CY2011 | 20120000349
His DD Form 214 shows his SSN as "x9x-xx-xxxx" and his DOB as "15 July 1947." The applicant listed his DOB as "15 July 1947" upon his induction into the AUS and this is the DOB that he used throughout his military service. He listed this DOB on several forms and authenticated many forms indicating this was his correct DOB.
