THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show:
* Army Achievement Medal (AAM) (3d Award)
* two Certificates of Achievement
* Combat Lifesaver Course
* 3d U.S. Infantry (The Old Guard) Regimental Orientation Program Training
2. The applicant states these items were not included on his DD Form 214 due to not being listed on his Enlisted Record Brief (ERB).
3. The applicant provides:
* 3d U.S. Infantry (The Old Guard) Regimental Orientation Program Certificate of Training
* Certificate of Achievement for support to the 50/20 Celebration
* Certificate of Participation in the 50/20 Celebration
* Combat Lifesaver Course Certificate of Training
* Certificate of Achievement, dated 29 August 2009
* Recommendation for Battalion Certificate of Achievement, dated 3 September 2009
* AAM (2nd Award) Certificate, dated 15 April 2011
* DA Form 638 (Recommendation for Award) for AAM (2nd Award)
CONSIDERATION OF EVIDENCE:
1. On 8 October 2008, the applicant enlisted in the Regular Army. He completed training, was awarded the military occupational specialty of 11B (Infantryman), and was promoted to pay grade E-4.
2. He submits a Certificate of Training showing he successfully completed the Combat Lifesaver Course during the period 10-14 December 2008.
3. He submits a 3d U.S. Infantry (The Old Guard) Regimental Orientation Program Certificate of Training showing he successfully completed 120 hours of training. The certificate does not show the completion date of the training.
4. He submits a Certificate of Achievement, dated 29 August 2009.
5. His record contains an AAM Certificate showing he was awarded the AAM by Permanent Orders 77-021, dated 18 March 2011, issued by Headquarters, 3d U.S. Infantry Regiment, for outstanding achievement for the period 18 February to 28 February 2011. He submits the accompanying DA Form 638 which shows in item 8 (Previous Awards) that he had one previous award of the AAM.
6. He submits a Certificate of Achievement and Certificate of Participation for support to the 50/20 Celebration in Kuwait, dated February 2011.
7. On 30 December 2011, he was discharged from active duty. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 lists the Army Achievement Medal (2nd Award). Item 14 (Military Education) of his DD Form 214 lists no military education.
8. Army Regulation 635-5 (Separation Documents) states in-service training courses successfully completed during the period of service covered by the DD Form 214 will be listed by the title, number of weeks, and year completed. This information is to assist the Soldier in job placement and counseling; therefore, do not list training courses for combat skills.
9. Army Regulation 635-5 provides that the DD Form 214 will reflect decorations, medals, badges, citations and campaign ribbons awarded or authorized for all periods of service. Certificates of Achievement, Letters of Appreciation, and similar documents are not recorded on the DD Form 214.
10. Army Regulation 350-1 (Army Training and Leader Development) states, in pertinent part, the combat lifesaver is a nonmedical Soldier trained to provide lifesaving measures beyond the level of self-aid or buddy-aid. This regulation also states combat lifesavers must be recertified every 12 months at the unit level.
11. The 3d U.S. Infantry Regiment (The Old Guard) is the official Escort to the President of the United States and our nation's Premier Memorial Affairs and Ceremonial Unit. All Soldiers assigned to the regiment attend the Regimental Indoctrination Program for 3 weeks prior to being assigned to a company within the regiment.
DISCUSSION AND CONCLUSIONS:
1. Permanent Orders Number 77-021 awarded him the AAM. A DA Form 638 shows he was recommended for the second award and that he had received one previous award of the AAM. A review of his records failed to reveal documentation for two previous awards of the AAM. Therefore, there is insufficient evidence to support adding the AAM (3d Award) to his DD Form 214.
2. He submits a Certificate of Training showing he successfully completed the Combat Lifesaver Course on 14 December 2008. Skills attained in the Combat Lifesavers Course are not strictly applicable to combat. Therefore, he is entitled to have this course added to his DD Form 214.
3. He submits a 3d U.S. Infantry (The Old Guard) Regimental Orientation Program Certificate of Training showing on an unidentified date he successfully completed 120 hours of training. However, there is no evidence indicating the instructional content would assist the Soldier in job placement and counseling. As such, it would not be appropriate to add this training to his DD Form 214.
4. Certificates of Achievement, Letters of Appreciation, and similar documents are not recorded on the DD Form 214. Therefore, there is no basis for adding the two Certificates of Achievement to his DD Form 214.
5. In view of the foregoing, he is entitled to have his records corrected as shown below.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to his DD Form 214 "Combat Lifesaver Course, 1 week, 2008."
2. The Board further determined the evidence presented was insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to adding the AAM (3d Award), the two Certificates of Achievement, and the 3d U.S. Infantry (The Old Guard) Regimental Orientation Program training to his DD Form 214.
