THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show "NO" in Item 17 (Member was provided complete dental examination and all appropriate dental services and treatment within 90 days prior to separation).

2.  The applicant states he received a dental examination on 21 November 2011 in conjunction with out-processing for separation.  After separation, he went to the Department of Veterans Affairs (VA) where he was shown the form indicating that all dental services had not been completed, which conflicts with the entry on his DD Form 214.

3.  The applicant provides copies of:

* DD Form 214
* Veteran's Outpatient Dental Treatment after Separation from Military Service, signed and dated 21 November 2011

CONSIDERATION OF EVIDENCE:

1.  On 1 August 2005, the applicant was ordered to active duty as a member of the U.S. Army Reserve in support of contingency operations.



2.  On 21 November 2011, the dental officer indicated on a preprinted form letter that the applicant had been provided a complete dental examination but that all dental services had not been completed.  Both the dentist and the applicant signed and dated this form.

3.  On 4 December 2011, the applicant was released from active duty.  He had completed 6 years, 4 months, and 4 days of creditable active duty service during this period.

4.  The applicant's DD Form 214 shows "YES" in Item 17 indicating that he had received a complete dental examination and all appropriate dental services.

5.  Army Regulation 635-5 (Separation Documents) provides detailed instructions for completing separation documents, including the DD Form 214.  The DD Form 214 is a summary of a Soldier's most recent period of continuous active duty.  It provides a brief, clear-cut record of active service at the time of release from active duty, retirement, or discharge.  Item 17 requires an entry indicating whether the service member received both a complete dental examination and all appropriate dental services.

DISCUSSION AND CONCLUSIONS:

1.  The applicant contends that his DD Form 214 should be corrected to show "NO" in Item 17.

2.  The available documentation clearly indicates the applicant did not receive all appropriate dental services within 90 days prior to his release from active duty, contrary to the entry made on his DD Form 214, which contains the entry "YES."

3.  In view of the above, the applicant's DD Form 214 should be corrected by showing the entry "NO" in item 17.

deleting the entry "YES" in Item 17 of his DD Form 214 and adding the entry "NO."
