THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that the records her deceased husband, a former service member (FSM), be corrected to show his Social Security Number (SSN) as ending in 5690.

2.  The applicant states his records incorrectly show a SSN ending in 5890.

3.  The applicant provides copies of the FSM's military identification card, Medicare Card, and death certificate.

CONSIDERATION OF EVIDENCE:

1.  At the time the FSM initially entered active duty the military utilized the Military Service Number and not the SSN for identification.  The earliest recorded use of an SSN, his security clearance application, utilitizes the SSN ending 5690.

3.  The FSM's records show the use of both the SSN ending in 5690 and 5890.

4.  His DD Form 214 (Report of Transfer or Discharge) utilizes the SSN ending 5890.

5.  The documents provided by the applicant show the FSM's SSN as ending 5690.


DISCUSSION AND CONCLUSIONS:

The preponderance of evidence shows the FSM's SSN was incorrectly shown on several of his service documents including his DD Form 214.  It is appropriate to correct the FSM's records to show the SSN ending in 5690.

As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending the FSM's DD Form 214 to show the SSN ending in 5690 as shown on his clearance application.
