THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected by adding the Joint Meritorious Unit Award (JMUA) (2d award), Armed Forces Service Medal (AFSM), and the Army Superior Unit Award (ASUA) (2d Award). 

2.  The applicant states that the awards were not included on his DD Form 214 because they had not been approved at the time of his retirement.

3.  The applicant provides copies of orders awarding the JMUA to Headquarters, Joint Task Force Haiti for the period 14 January to 1 June 2010, a memorandum from the Under Secretary of Defense awarding the AFSM to service members deployed in support of Operation Unified Response in Haiti, and orders awarding the ASUA to Main Command Post, Headquarters, U.S. Army South; Operational Command Post, Headquarters, U.S. Army South; and Special Troops Battalion, Headquarters, U.S. Army South.

CONSIDERATION OF EVIDENCE:

1.  The applicant was a Regular Army lieutenant colonel assigned to Headquarters, U.S. Army South at Fort Sam Houston, Texas for duty as the Assistant Chief of Staff – G1 (Manpower) when he deployed to Haiti from 20100312 – 20100521.



2.  His officer evaluation report ending on 3 June 2010 indicates that he deployed as the Joint Task Force – Haiti J1 for Operation Unified Response and that he served as the commanding general’s personal representative to the U.S. Southern Command (SOUTHCOM) commander. 

3.  The applicant’s official records show he was assigned to the units presented the JMUA, the AFSM, and the ASUA during the prescribed timeframe indicated.

4.  On 31 December 2010 the applicant was retired.  His DD Form 214 does not reflect the awards indicated in the orders provided by the applicant.

DISCUSSION AND CONCLUSIONS:

1.  The applicant’s unit was awarded the JMUA, the AFSM, and the ASUA during the period he served with the unit.

2.  Accordingly, he is entitled to have the JMUA (2d Award), AFSM, and the ASUA (2d Award) added to his DD Form 214 at this time.

BOARD DETERMINATION/RECOMMENDATION:

1. The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the JMUA (2d Award), AFSM, and the ASUA (2d Award) added to his 
DD Form 214.

2.  The Board wants the applicant and all others to know the sacrifices he made in service to the United States during the Global War on Terrorism are deeply 


appreciated.  The applicant and all Americans should be justifiably proud of his service in arms.
