THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Bronze Star Medal (3d Award), Afghanistan Campaign Medal with three bronze service stars, and Meritorious Unit Commendation (2nd Award).
2. He states the awards were awarded/received after the completion of his DD Form 214.
3. He provides:
* DD Form 214
* Bronze Star Medal Certificate with permanent orders number
* U.S. Army Human Resources Command (HRC) Permanent Orders 133-01
CONSIDERATION OF EVIDENCE:
1. His records show he entered active duty as a commissioned officer on 22 May 1996.
2. A DA Form 67-9 (Officer Evaluation Report) contained in his records shows he was assigned to Headquarters and Headquarters Company, 3d Battalion,
101st Aviation Regiment, 101st Airborne Division (Air Assault), during the rating period 14 February 2003 through 13 February 2004.
3. Headquarters, Department of the Army General Orders 3, dated 6 November 2007, awarded the Meritorious Unit Commendation to the 3d Battalion, 101st Aviation Regiment, 101st Airborne Division (Air Assault), and its subordinate units for the period 1 July 2003-26 January 2004.
4. Permanent Orders 091-006D, dated 31 March 2004, published by Headquarters, 101st Airborne Division (Air Assault), awarded him the Bronze Star Medal for the period 19 March 2003 to 1 March 2004.
5. Permanent Orders 131-004, dated 11 May 2007, awarded him the Bronze Star Medal for the periods: 26 May 2006 to 4 August 2006 and 27 November 2006 to 20 February 2007.
6. A DA Form 67-9 contained in his records shows he was assigned to Company B, 2nd Battalion, 160th Special Operations Aviation Regiment (Airborne), during the rating period 2 May 2008 through 14 June 2010.
7. Permanent Orders 133-01, dated 13 May 2011, published by HRC, awarded the Meritorious Unit Commendation to Headquarters and Headquarters Company, 2d Battalion, 160th Aviation Regiment and its subordinate units for the period of service of 1 October 2009 to 30 September 2010.
8. He was discharged from active duty on 14 June 2011. His DD Form 214 shows he completed 15 years and 23 days of active duty.
9. His DD Form 214 shows, in pertinent part, the Bronze Star Medal
(2nd Award), Afghanistan Campaign Medal with campaign star (2nd Award), and Meritorious Unit Commendation.
10. His DD Form 214 shows he served in Afghanistan during the periods:
* 24 April 2006 to 14 August 2006
* 26 November 2006 to 20 February 2007
* 3 May 2009 to 15 May 2009
* 23 September 2009 to 6 December 2009
* 20 February 2010 to 7 April 2010
* 24 July 2010 to 4 September 2010
11. Permanent Orders 341-010, dated 7 December 2011, awarded him the Bronze Star Medal for the periods: 21 September 2009 to 6 December 2009, 23 February 2010 to 7 April 2010, and 30 July 2010 to 4 September 2010.
12. Army Regulation 600-8-22 (Military Awards) states the Afghanistan Campaign Medal is awarded to members who have served in direct support of Operation Enduring Freedom (OEF). The area of eligibility encompasses all the land area of the country of Afghanistan and all air spaces above the land area. The period of eligibility is on or after 24 October 2001 to a future date to be determined by the Secretary of Defense or the cessation of OEF. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved designated Afghanistan campaigns are:
* Liberation of Afghanistan (11 September 2001-30 November 2001)
* Consolidation I (1 December 2001-30 September 2006)
* Consolidation II (1 October 2006-30 November 2009)
* Consolidation III (1 December 2009-30 June 2011)
* Transition (1 July 2011-date to be determined)
DISCUSSION AND CONCLUSIONS:
1. Permanent Orders 341-010 awarded him the Bronze Star Medal. His records show he received two previous awards of the Bronze Star Medal. Therefore, he is entitled to have his DD Form 214 amended to show the Bronze Star Medal
(3d Award).
2. His DD Form 214 shows he was authorized the Afghanistan Campaign Medal with two bronze service stars. Evidence show he participated in three campaigns in Afghanistan during his military service. Therefore, he is entitled to have his DD Form 214 amended to show the Afghanistan Campaign Medal with three bronze service stars.
3. Headquarters and Headquarters Company, 2d Battalion, 160th Aviation Regiment and Headquarters and Headquarters Company, 3d Battalion, 101st Aviation Regiment, 101st Airborne Division (Air Assault) were awarded the Meritorious Unit Commendation during a period when he was assigned to those units. Therefore, he is entitled to have his DD Form 214 amended to show the Meritorious Unit Commendation (2nd Award).
4. In view of the foregoing, the applicant is entitled to have his records corrected as shown below.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 to show the Bronze Star Medal (3d Award) in lieu of the 2d Award, Afghanistan Campaign Medal with three bronze service stars in lieu of the 2d Award, and Meritorious Unit Commendation (2nd Award) in lieu of the 1st Award.
