THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, reconsideration of his request for correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show his social security number as "XXX-XX-9067" instead of "XXX-XX-7108."
2. The applicant states that a typographical error resulted in his selective service number being entered on his DD Form 214. He discovered this error when he applied for veterans' benefits.
3. The applicant provides copies of his DD Form 214, his birth certificate, a letter from the Social Security Administration, and his social security card.
CONSIDERATION OF EVIDENCE:
1. Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the Army Board for Correction of Military Records (ABCMR) in Docket Number AR20100019868 on 3 February 2011.
2. The applicant's record contains a DD Form 47 (Record of Induction) which shows he was inducted into the Army of the United States on 26 July 1966. This form also shows his selective service number as "XX-XX-X7-108."
3. A thorough review of documents contained in the applicant's military personnel records shows his social security number appears as both
"XXX-XX-9067" and "XXX-XX-7108" on several documents throughout his tenure of service.
a. His social security number appears as "XXX-XX-9067" on documents including:
* DA Form 3027 (U.S. Army Request for National Agency Check)
* DA Form 41 (Record of Emergency Data)
* military occupational specialty orders
* promotion orders
* letter orders discharging him from the U.S. Army Reserve
b. His social security number appears as "XXX-XX-7108" on documents including:
* reassignment orders
* promotion orders
* orders releasing him from active duty
4. Item 3 (Social Security Number) of the applicant's DD Form 214 shows his social security number as "XXX-XX-7108." Item 10a (Selective Service Number) shows his selective service number as "XX-XX-X7-108."
5. The applicant provides a letter from the Social Security Administration and a copy of his social security card issued by the Social Security Administration which show his social security number as "XXX-XX-9067."
6. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. This regulation establishes standardized policy for preparation of the DD Form 214. It states the complete name and social security number of the separating service member will be entered on the DD Form 214.
DISCUSSION AND CONCLUSIONS:
1. The applicant's contention that his DD Form 214 should be corrected to show his social security number as "XXX-XX-9067" was carefully considered and determined to have merit.
2. The evidence clearly shows the applicant's selective service number was erroneously entered in lieu of his social security number on numerous documents. Although the applicant's social security number appears as both "XXX-XX-9067" and "XXX-XX-7108" in his military personnel records, the Social Security Administration confirms the applicant's social security number is
"XXX-XX-9067." Therefore, he is entitled to have his DD Form 214 corrected to show his social security number as "XXX-XX-9067."

BOARD DETERMINATION/RECOMMENDATION:

As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 to reflect his social security number as it is shown on his social security card.
