THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Combat Infantryman Badge (CIB) and a second award of the Army Commendation Medal (ARCOM).
2. The applicant states he left the Army before receiving his orders for the CIB. He was also awarded an ARCOM at the expiration of his term of service that is not listed on his DD Form 214 and he does not have the orders.
3. The applicant provides orders for the CIB.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 2 November 2006 and upon completion of initial entry training, he was awarded military occupational specialty 11B (Infantryman).
2. Permanent Orders Number 054-003, issued by Headquarters, 2nd Stryker Brigade, Combat Team, 2nd Infantry Division, Joint Base Lewis-McChord, WA, dated 23 February 2011, awarded him the CIB for being engaged by or engaging the enemy on 22 June 2010.
3. There are no orders or any other evidence in his Official Military Personnel File (OMPF) that shows he was awarded a second award of the ARCOM.
4. He was honorably released from active duty on 4 April 2011. His DD Form 214 shows one award of the ARCOM. It does not show the CIB.
5. Army Regulation 600-8-22 (Military Awards) states the ARCOM may be awarded to any member of the Armed Forces of the United States who, while serving in any capacity with the Army after 6 December 1941, distinguishes himself or herself by heroism, meritorious achievement, or meritorious service. As with all personal decorations, formal recommendations, approval through the chain of command, and announcement in orders are required.
DISCUSSION AND CONCLUSIONS:
1. Permanent orders awarded the applicant the Combat Infantryman Badge. Therefore, his DD Form 214 should be corrected to show this award.
2. There is no evidence in the applicant's OMPF and he did not provide any evidence that shows he was awarded a second award of the ARCOM. Therefore, there is no basis for granting this portion of his request.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to his DD Form 214 the Combat Infantryman Badge.
2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief. As a result, the Board recommends
denial of so much of the application that pertains to adding a second award of the Army Commendation Medal to his DD Form 214.
