THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show her award of the Iraq Campaign Medal (ICM) with a campaign star.
2. She states her award of the ICM was not entered on her DD Form 214 at the time of her retirement.
3. She provides her DD Form 214 and Enlisted Record Brief (ERB).
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 7 January 1992 and was trained as a motor transport operator. She remained on active duty through a series of continuous reenlistments. She was promoted to the pay grade of E-7 on 1 July 2002.
2. She deployed to Kuwait from 20021106 20030627 and to Iraq in one campaign from 20041118 20051118.
3. On 31 January 2012, she was retired.
4. Her DD Form 214 does not show award of the ICM.
5. Army Regulation 600-8-22 (Military Awards) states the ICM is awarded to members who have served in direct support of Operation IraqiFreedom (OIF). The ICM period of eligibility is on or after 19 March 2003 to 31 December 2011. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved campaigns includes the:
* Iraqi Governance (29 June 2004-15 December 2005)
DISCUSSION AND CONCLUSIONS:
The applicant deployed to Iraq from 20041118 20051118. Therefore, she is entitled to award of the ICM with one bronze service star.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to her DD Form 214 the Iraq Campaign Medal with one bronze service star.
2. The Board wants the applicant and all others to know that the sacrifices she made in service to the United States during the Global War on Terrorism are deeply appreciated. The applicant and all Americans should be justifiably proud of her service in arms.
