THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected by accounting for his overseas service in Iraq in item 12f (Foreign Service); and by adding all awards due based on this service.   

2.  The applicant states item 12f of his DD Form 214 does not show his foreign service and the awards he earned for this service should have been included in the DD Form 214.  

3.  The applicant provides his DD Form 214 in support of the application.  

CONSIDERATION OF EVIDENCE:

1.  The record shows the applicant enlisted in the Regular Army on 
15 September 2005, and was trained in and awarded military occupational specialty (MOS) 91B (Wheel Vehicle Mechanic).  

2.  On 8 March 2010, the applicant was honorably released from active duty, in the rank of specialist/E-4, after completing 4 years, 5 months, and 25 days of active military service.  The DD Form 214 he was issued at the time contains the entry "0000  00  00" in item 12f.  Item 18 (Remarks) contains an entry confirming Iraq service from 10 October 2006 through 10 October 2007.  There are no Iraq or Global War on Terror (GWOT) service awards listed on the DD  Form 214.



3.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army.  It also establishes standardized policy for preparing and distributing the DD Form 214.  Chapter 2 of contains item-by-item instructions for preparation of the DD Form 214.  The instructions for item 12f are to add all overseas service completed during the period covered by the DD Form 214.  The instructions for item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) are to add all awards earned during military service.  

4.  Army Regulation 600-8-22 (Military Awards) contains the Army's awards policy.  Paragraph 2-17 contains guidance on the Iraq Campaign Medal (ICM).  It states, in pertinent part, that it is authorized for service in Iraq on or after 
19 March 2003, to a date to be determined by the Secretary of Defense.  It also states that a bronze service star is authorized with the ICM for each campaign a member is credited with participating in while serving in Iraq.  During the applicant's tenure of assignment in Iraq, participation credit was granted for the following two campaigns:

* National Resolution (16 December 2005-9 January 2007)
* Iraqi Surge (10 January 2007-31 December 2008)

5.  Paragraph 2-19 of the awards regulation contains guidance on the Global War on Terrorism Service Medal (GWOTSM).  It states it is authorized to members who served on active duty in support of GWOT operations outside of an authorized area of eligibility for the Global War on Terrorism Expeditionary Medal (GWOTEM) for 30 consecutive or 60 nonconsecutive days between 9 September 2001 through a date to be determined by the Secretary of Defense.  

DISCUSSION AND CONCLUSIONS:

1.  The applicant’s request to account for his Iraq service in item 12f and to add all awards due based on his Iraq service to his DD Form 214 has been carefully considered and found to have merit.  

2.  By regulation, all foreign service completed during the period covered by a 
DD Form 214 will be entered in item 12f, and all awards earned during military service will be entered on the DD Form 214.  



3.  The evidence of record confirms the applicant served in Iraq for 1 year and 1 day from 10 October 2006 through 10 October 2007, as evidenced by the entry in item 18 of his DD Form 214.  Therefore, it would be appropriate to correct 
item 12f accordingly.  

4.  Further, based on the applicant’s service and campaign participation in Iraq, he is eligible for the ICM with 2 bronze service stars.  In addition, based on his qualifying active duty service after 11 September 2001, he is also eligible for the GWOTSM.  Therefore, it would be appropriate to add these awards to his record and DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending the following items of his DD Form 214 as indicated:

	a.  item 12f – delete the current entry and replace it with the entry "0001  00  01" and 

	b.  item 13 – add the Iraq Campaign Medal with two bronze service stars and Global War on Terrorism Service Medal.
