THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty), dated 15 December 1998, be corrected to show that he served 2 years and 5 months of foreign service in block 12f and his deployments to Iraq and Kuwait in block 18.
2. The applicant states that he served in Germany from 19870612 19890601 (24 months), in Iraq from 19910201 19910531 (4 months), and in Kuwait from 19911201 19911231 (1 month), and he desires this information to be reflected on his DD Form 214.
3. The applicant provides a copy of his DD Form 214, memoranda verifying his deployments, an overseas tours credit listing, and a letter of commendation.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 28 January 1987. He completed his one-station unit training as a light weapons infantryman at Fort Benning, Georgia and was transferred to Germany on 12 June 1987. He departed Germany on 1 June 1989 and was transferred back to Fort Benning where he underwent Ranger training and was then transferred to the Ranger Battalion at Hunter Army Airfield, Georgia.
2. He deployed to Iraq from 19910217 19910404 (1 month and 18 days) and to Kuwait from 19911206 19911212 (7 days).
3. On 15 December 1998, he was honorably discharged at Fort Benning, Georgia upon graduation from Officer Candidate School (OCS) to accept a commission in the Army. He had served 11 years, 10 months and 18 days of active service and his DD Form 214 issued at the time of discharge shows no foreign service in block 12f and no deployments in block 18.
4. The applicants records show that he served 24 months in Germany, 1 month and 18 days in Iraq, and 7 days in Kuwait; this foreign service equals 2 years,
1 month, and 24 days.
5. On 16 December 1998, he was commissioned as a second lieutenant and remained on active duty until he was retired as a special forces major on
31 August 2011 and placed on the Retired List effective 1 September 2011. He had served 24 years, 7 months, and 3 days of active service.
6. Army Regulation 635-5 (Personnel Separations - Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states that for:
a. block 12f, enter the total amount of foreign service completed during the period covered by the DD Form 214.
b. block 18, for an active duty Soldier deployed to a foreign country with his or her unit during their continuous period of active service, enter the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)."
DISCUSSION AND CONCLUSIONS:
1. The applicants contention that his foreign service and deployments should be added to his DD Form 214, dated 15 December 1998, has been noted and found to have merit.
2. The applicant served 24 months in Germany, 1 month and 18 days in Iraq, and 7 days in Kuwait; this foreign service equals 2 years, 1 month, and 24 days for the period covered by the DD Form 214. Accordingly, it should be added to block 12f of his DD Form 214.
3. Additionally, his deployments to Iraq during the period 19910217 19910404 and to Kuwait from 19911206 19911212 should be added to block 18 of his DD Form 214.
