THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that his report of separation (DD Form 214) be corrected to reflect his foreign service in Kuwait and Iraq.
2. The applicant states that he entered Kuwait on 14 February 2003 and entered Iraq on 19 March 2003; however, his service in Southwest Asia is not reflected on his DD Form 214.
3. The applicant provides a copy of an Army Commendation Medal (ARCOM) he received for his service during Operation Iraqi Freedom, a copy of his DD Form 214 and a copy of his Post-Deployment Health Assessment.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 16 November 2000 for a period of 4 years, training as a radio operator and maintainer and assignment to Fort Hood, Texas.
2. He completed his basic combat training at Fort Benning, Georgia and his advanced individual training at Fort Gordon, Georgia before being transferred to Fort Hood. He was assigned to the 615th Aviation Support Battalion, 1st Cavalry Division and was advanced to the pay grade of E-3 on 1 July 2001.
3. On 14 February 2003, he deployed to Kuwait with the 1st Battalion, 227th Aviation Regiment in support of Operation Enduring Freedom. He deployed to Iraq with the unit on 21 March 2003 in support of Operation Iraqi Freedom and remained in Iraq until 21 May 2003, when the 227th Aviation Regiment returned to Fort Hood and the applicant was returned to his assigned unit.
4. On 12 March 2004, he was honorably discharged under the provisions of Army Regulation 635-200, chapter 18, due to weight control failure. He had served 3 years, 3 months, and 27 days of total active service. His DD Form 214 issued at the time of his discharge reflects no foreign service in block 12f and contains no indication/remarks in item 18 to show his service in Southwest Asia during the period of 14 February to 21 May 2003.
5. The applicant was awarded an ARCOM on 25 August 2003 for outstanding performance in support of Operation Iraqi Freedom during the period of 19 March to 21 May 2003.
DISCUSSION AND CONCLUSIONS:
1. The applicant?s contention that his service in Southwest Asia is not reflected on his DD Form 214 has been noted and found to have merit.
2. The applicant deployed to Kuwait on 14 February 2003 in support of Operation Enduring Freedom and then to Iraq on 21 March 2003 in support of Operation Iraqi Freedom, where he remained until 21 May 2003, when he was deployed back to Fort Hood.
3. Accordingly, his DD Form 214 should be corrected to reflect his 3 months and 8 days of foreign service in block 12f and his participation in Operations Enduring Freedom and Iraqi Freedom during the period of 14 February 2003 to 21 May 2003.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by showing that he served 3 months and 8 days of foreign service and that he participated in Operations Enduring Freedom and Iraqi Freedom during the period of 14 February to 21 May 2003.
