THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Order of Military Medical Merit certificate.

2.  The applicant states this certificate was not processed until after she came off mobilization orders and was retired from the U.S. Army Reserve (USAR).

3.  The applicant provides her DD Form 214, DD Form 215 (Correction to DD Form 214), three orders, and a certificate.

CONSIDERATION OF EVIDENCE:

1.  The applicant's records show she was a USAR colonel in the U.S. Army Nurse Corps.  She was ordered to active duty in support of Operation Enduring Freedom and entered active duty on 6 February 2005.

2.  She was honorably released from active duty on 29 November 2010 by reason of completion of required active service.  The DD Form 214 she was issued shows she completed 5 years, 9 months, and 24 days of creditable active service.

3.  The applicant provides a certificate, dated 9 June 2011, wherein it stated membership in the Order of Military Medical Merit was vested in her by recommendation of the advisory council.  The certificate is signed by a retired general.
4.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It establishes standardized policy for the preparation of the DD Form 214 and DD Form 215.  In pertinent part, it states the DD Form 214 is a synopsis of the Soldier’s most recent period of continuous active duty.  It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement or discharge.  In item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) list all awards for all periods of service.

DISCUSSION AND CONCLUSIONS:

The DD Form 214 is a record of a Soldier's service at the time of separation from active duty.  There is no provision for adding membership certificates to the DD Form 214.  Therefore, she is not entitled to the requested relief.
