THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his 2005/2006 deployment dates in Iraq.

2.  The applicant states:

* His DD Form 214 only shows one deployment to Iraq (2008/2009) but he also deployed to Iraq in 2005
* His reenlistment paperwork shows he reenlisted in Iraq on 30 December 2005
* He had multiple tours into Iraq

3.  The applicant provides his enlistment and reenlistment contracts.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 22 July 2003 for a period of 
3 years.  He served as an infantryman in military occupational specialty 11B.  On 29 December 2005, he was honorably discharged for immediate reenlistment.  On 30 December 2005, he reenlisted for a period of 6 years.  His reenlistment contract shows his place of reenlistment was Iraq.  On 19 April 2010, he was discharged under honorable conditions (a general discharge).



2.  Item 18 of his DD Form 214 shows the entry "SERVICE IN IRAQ 20080920-20090830."

3.  The Defense Finance and Accounting Service (DFAS) confirmed the applicant received hostile fire/imminent danger pay and combat zone tax exclusion from 
1 February 2005 through 12 January 2006 for service in Kuwait.

DISCUSSION AND CONCLUSIONS:

His DD Form 214 shows he served in Iraq in 2008/2009.  However, DFAS confirms he served in Kuwait/Iraq from 1 February 2005 to 12 January 2006.  Therefore, this information is accepted as sufficient evidence with which to correct item 18 of his DD Form 214 to show a second deployment in Kuwait/Iraq from 1 February 2005 to 12 January 2006.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the entry "SERVICE IN KUWAIT/IRAQ FROM 20050201-20060112" in item 18 of his DD Form 214.
