THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 3 August 1994 to show the award of the Army Good Conduct Medal.
2. The applicant states he received an honorable discharge on 3 August 1994, after being granted a 4-year Green to Gold Army college scholarship. Further, Army Regulation 600-8-22 (Military Awards), paragraph 4-5c states, for the first award only, upon termination of service on or after 27 June 1950, of less than
3 years but more than 1 year.
3. The applicant provides:
* his DD Form 214 for the period ending 3 August 1994
* a Certificate of Promotion to specialist, dated 1 April 1993
* a Certificate of Achievement for being his unit's Soldier of the Month for
30 September 1993
* a Certificate of Recognition, dated 2 March 1993
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 16 October 1991. He was trained in and awarded military occupational specialty 91E (Dental Specialist).
2. On 3 August 1994, he was honorably discharged in the rank/grade of specialist (SPC)/E-4 for the purpose of entering an officer training program. He was transferred to the U.S. Army Reserve (USAR) Control Group (Reserve Officers' Training Corps (ROTC).
3. Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) of the DD Form 214 he was issued at the time does not show award of the Army Good Conduct Medal.
4. He subsequently completed the officer training program and he was appointed as a second lieutenant (2LT)/O-1 in the District of Columbia Army National Guard (DCARNG), effective 24 August 2003. On 2 December 2005, he was ordered to active duty in support of Operation Iraqi Freedom (OIF). He served in Iraq from 19 January to 10 December 2006. On 30 December 2006 he was honorably released from active duty and he returned to the DCARNG.
5. Item 13 of the DD Form 214 for this period of service shows the:
* Bronze Star Medal
* Army Reserve Components Achievement Medal
* National Defense Service Medal (2nd Award)
* Global War on Terrorism Service Medal
* Iraq Campaign Medal
* Army Service Ribbon
* Overseas Service Ribbon
* Armed Forces Reserve Medal with "M" Device
* German Armed Forces Achievement Badge in Gold
6. The applicant was subsequently transferred to the Maryland Army National Guard (MDARNG) where is currently serving in the rank/grade of captain (CPT)/O-3.
7. The applicant's enlisted service record is not available for review; however, his interactive Personnel Electronic Records Management System (iPERMS) file contains Permanent Orders 302-023, issued by Third Army, U.S. Army Central Area Support Group Qatar, dated 3 November 2011, that awarded him the Army Good Conduct Medal (1st Award) for the period 16 October 1991 through
3 August 1994. A copy of these orders will be provided to the applicant with these Proceedings.
8. Army Regulation 600-8-22 (Military Awards) states the Iraq Campaign Medal is awarded to members who have served in direct support of OIF. The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 to a future date to be determined by the Secretary of Defense or the cessation of OIF. A bronze service star is authorized for wear with this medal for participation in each credited campaign. During the applicant's service in support of OIF, he participated in the National Resolution (16 December 2005 - 9 January 2007) campaign.
9. Army Regulation 635-5 (Personnel Separations - Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states that awards and decorations for all periods of service in the priority sequence specified in Army Regulation 600-8-22 will be entered on the DD Form 214. Each entry will be verified by the Soldiers records.
DISCUSSION AND CONCLUSIONS:
1. Permanent orders awarded the applicant the Army Good Conduct Medal
(1st Award) for the period 16 October 1991 through 3 August 1994. Therefore, his DD Form 214 for the period ending 3 August 1994 and 30 December 2006 should be corrected to show this award.
2. The applicant participated in one campaign phase during his service in Iraq. Therefore, he is entitled to one bronze service star for wear on his already-awarded Iraq Campaign Medal and correction of his DD Form 214 for the period ending 30 December 2006 to show this bronze service star.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. deleting from item 13 of his DD Form 214 for the period ending
30 December 2006 the Iraq Campaign Medal;
b. adding to item 13 of his DD Forms 214 for the period ending 3 August 1994 and 30 December 2006 the Army Good Conduct Medal (1st Award);
c. adding to item 13 of his DD Form 214 for the period ending 30 December 2006 the Iraq Campaign Medal with one bronze service star
d. issuing him the Army Good Conduct Medal set as a result of this correction.
