THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Valorous Unit Award.

2.  The applicant states the award was authorized after his discharge.

3.  The applicant provides Permanent Order 263-11, U.S. Army Human Resources Command, Fort Knox, KY, dated 18 September 2008.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Minnesota Army National Guard on                   11 December 2001.  He entered active duty, effective 15 November 2003, in support of Operation Iraqi Freedom.
 
2.  On 24 April 2005, he was honorably released from active duty.  His DD Form 214 shows he completed 1 year, 5 months, and 10 days of active service during the period covered, including 1 year and 7 days of foreign service.

3.  Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 shows he was awarded the Army Commendation Medal, National Defense Service Medal, Global War on Terrorism Expeditionary Medal, Global War on Terrorism Service Medal, Armed Forces Reserve Medal with “M” Device, and the Army Lapel Button.  

4.  Item 18 (Remarks) of his DD Form 214 shows he was deployed to Iraq from 13 March 2004 to 16 March 2005.  

5.  His records contain copies of

	a.  Certificate of Achievement awarded to the applicant for meritorious and outstanding duty performance during Operations Iraqi Freedom I and II as a member of Task Force 4-5 Air Defense Artillery, 1st Cavalry Division from March 2004 to March 2005.

	b.  Permanent Orders, 045-59, National Guard Training and Community Center, Headquarters, 34th Infantry “Red Bull” Division, dated 14 February 2006, awarding him the Combat Action Badge.  The period of service is shown as 
10 November 2004.

6.  In support of his request, he provides a copy of Permanent Order 263-11, U.S. Army Human Resources Command, Fort Knox, KY, dated 18 September 2008, announcing the 4th Battalion, 5th Air Defense Artillery, was awarded the Valorous Unit Award for the period 7 April 2004 to 11 April 2004.

DISCUSSION AND CONCLUSIONS:  

1.  The available evidence shows the applicant's unit in Iraq was awarded the Valorous Unit Award when he was assigned to the unit.

2.  His records contain orders awarding him the Combat Action Badge. 

3.  These awards should be listed on his DD Form 214.

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending item 13 of his DD Form 214 to add the Combat Action Badge and Valorous Unit Award.
