THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to add all missing awards and his two tours of duty in Iraq.

2.  The applicant states his DD Form 214 is incorrect.

3.  The applicant provides no additional documents.

CONSIDERATION OF EVIDENCE:

1.  The applicant was a Regular Army Soldier with service from 18 March 2004 through 18 August 2010 when he was honorably released from active duty by reason of completion of required active service under the provisions of Army Regulation 635-200, chapter 4.

2.  The applicant's DD Form 214 does not indicate any Iraq deployments in item 18 (Remarks).

3.  The Defense Finance and Accounting Service (DFAS) verified the applicant received hostile fire/imminent danger pay and combat zone tax exclusion for service in Kuwait on two occasions:

* from 22 December 2004 through 22 September 2005
* from 3 November 2006 through 16 December 2007

4.  The applicant's Enlisted Record Brief shows he was awarded the following medals:

* Army Good Conduct Medal
* National Defense Service Medal
* Iraq Campaign Medal (ICM) with campaign [bronze service] star
* Global War on Terrorism Expeditionary Medal (GWOTEM)
* Global War on Terrorism Service Medal
* Army Service Ribbon

5.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It establishes standardized policy for the preparation of the DD Form 214.  The regulation states for item 18, for an active duty Soldier deployed with his or her unit during their continuous period of active service, enter "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)."

6.  Military Personnel Message Number 08-123, subject:  Afghanistan Campaign Medal (ACM) and ICM Campaign Stars, provides that:

* service stars recognize Soldiers for participating in designated campaign phases within the areas of eligibility for the ACM and ICM and provide tangible recognition for the sacrifices and contributions they have made in support of the Global War on Terror in Afghanistan and Iraq
* ICM campaign phases/inclusive periods are:

* Liberation of Iraq/19 March 2003-1 May 2003
* Transition of Iraq/2 May 2003-28 June 2004
* Iraqi Governance/29 June 2004-15 December 2005
* National Resolution/16 December 2005-9 January 2007
* Iraqi Surge/10 January 2007-31 December 2008
* Iraqi Sovereignty/1 January 2009-31 August 2010
* New Dawn/1 September 2010-date to be determined

* Soldiers qualified for the GWOTEM by reason of service between 19 March 2003 and 30 April 2005 in an area for which the ICM was subsequently authorized shall remain qualified for that medal, but under no condition shall a Soldier receive the ACM, GWOTEM, ICM, or Armed Forces Expeditionary Medal for the same action, time period, or service
* the service star is not authorized for wear on the GWOTEM

DISCUSSION AND CONCLUSIONS:

1.  The applicant served in Iraq from 22 December 2004 through 22 September 2005 and was awarded the GWOTEM.  He served in Iraq a second time from 3 November 2006 through 16 December 2007 and was awarded the ICM with one bronze service star.  He is authorized two bronze service stars.  He is entitled to have these awards added to his DD Form 214.

2.  Army Regulation 635-5 prescribes that an entry will be made in item 18 of the DD Form 214 for an active duty Soldier deployed with his or her unit during their continuous period of active service.  The applicant participated in two deployments and these should be reflected in item 18 of his DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

* adding the GWOTEM and ICM with two bronze service stars to item 13 of his DD Form 214
* adding the entry "SERVICE IN KUWAIT/IRAQ FROM 20041222-20050922 AND 20061103-20071216" to item 18 of his DD Form 214
