THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the:
* Army Commendation Medal
* Air Force Space Badge
2. The applicant states he received the approved awards after his DD Form 214 was issued.
3. The applicant provides:
* DA Form 638 (Recommendation for Award) and certificate awarding him the Army Commendation Medal
* Certificate and letter awarding him the Air Force Space Badge
CONSIDERATION OF EVIDENCE:
1. On 21 June 2006, the applicant enlisted in the Regular Army and held military occupational specialty 14J (Air Defense Command, Control, Communications, Computers, and Intelligence Tactical Operations Center Enhanced Operator/Maintainer).
2. On 21 November 2010, he was honorably released from active duty and transferred to the U.S. Army Reserve Control Group to complete his remaining service obligation. Item 13 (Awards, Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 shows the:
* Army Achievement Medal (2nd Award)
* Army Good Conduct Medal
* National Defense Service Medal
* Global War on Terrorism Service Medal
* Korean Defense Medal
* Noncommissioned Officer Professional Development Ribbon
* Army Service Ribbon
* Overseas Service Ribbon
3. Permanent Orders Number 270-02 issued by Headquarters, 1st Space Brigade, Patterson Air Force Base, awarded him the Army Commendation Medal for meritorious service from 20 May 2009 to 21 November 2010.
4. A memorandum, issued by the U.S. Army Space and Missile Defense Command, Patterson Air Force Base, CO, authorized the applicant acceptance and wear of the Air Force Space Badge (Basic).
DISCUSSION AND CONCLUSIONS:
1. Permanent orders awarded him the Army Commendation Medal which is not shown on his DD Form 214; therefore, he is authorized correction of his DD Form 214 to show it.
2. The U.S. Army Space and Missile Defense Command, Patterson Air Force authorized him award and wear of the Air Force Space Badge (Basic) which is not shown on his DD Form 214; therefore, he is authorized correction of his
DD Form 214 to show it.
