THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction to his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Army Good Conduct Medal, Combat Action Badge, and Army Achievement Medal.
2. He states, in effect, when he was demobilized in 2010 he was issued a DD Form 214 that did not list all of his awards because they did not have all of his records. He was told that he could get a DD Form 215 (Correction to DD Form 214) when he had all of his records to update it later.
3. He provides:
* Memorandum authorizing award of the Army Good Conduct Medal, dated 14 October 2005
* Combat Action Badge orders, dated 23 November 2005
* Army Achievement Medal Certificate, dated 24 June 2007
* DD Form 214 ending 5 May 2010
CONSIDERATION OF EVIDENCE:
1. The applicant's military records show he enlisted in the U.S. Army Reserve on 18 December 2002. He was ordered to active duty in support of Operation Iraqi Freedom and entered active duty on 19 January 2004. He was honorably released from active duty (REFRAD) on 21 March 2005.
2. Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) of his DD Form 214 lists the:
* Driver Badge with Driver-Wheeled Vehicle Clasp
* Army Lapel Button
* Army Commendation Medal
* National Defense Service Medal
* Global War on Terrorism Service Medal
* Global War on Terrorism Expeditionary Medal
* Armed Forces Reserve Medal with "M" Device
* Army Service Ribbon
3. Item 18 (Remarks) of his DD Form 214 contains the entry "SERVICE IN OPERATION IRAQI FREEDOM FROM 20040303-20050223."
4. A Memorandum, Subject: Army Good Conduct Medal, dated 14 October 2005, authorized a roster of Soldiers, to include him, the Army Good Conduct Medal (1st Award) for their periods of service while on active duty. He was authorized the award for his service from 19 January 2004 through 20 March 2005.
5. Permanent Order 327-001, dated 23 November 2005, was issued awarding him the Combat Action Badge for service from 17 March 2004 through 22 February 2005.
6. He provided a copy of a certificate, dated 24 June 2007, awarding him the Army Achievement Medal for meritorious service during the 2007 Pacific Warrior Exercise from 6 to 28 June 2007, issued by Permanent Order 175-003, dated 24 June 2007.
7. He was again ordered to active duty in support of Operation Iraqi Freedom and entered active duty on 5 April 2009. He was honorably REFRAD on 5 May 2010 and was transferred to the Individual Ready Reserve.
8. Item 13 of his DD Form 214 lists the following:
* Iraq Campaign Medal with four bronze service stars
* Army Commendation Medal
* National Defense Service Medal
* Global War on Terrorism Service Medal
* Global War on Terrorism Expeditionary Medal
* Army Service Ribbon
* Overseas Service Ribbon (2nd Award)
* Armed Forces Reserve Medal with "M" Device
* Driver and Mechanic Badge with Driver-Wheeled Vehicle (S) Clasp
9. Item 18 (Remarks) of this DD Form 214 contains the entry "SERVICE IN KUWAIT/IRAQ 20090715-20100321."
DISCUSSION AND CONCLUSIONS:
1. The evidence of record shows the applicant was awarded the Army Good Conduct Medal (1st Award) and Combat Action Badge during his period of active duty from 19 January 2004 through 21 March 2005. Therefore, based on this evidence, he is entitled to correction to his DD Form 214 ending 21 March 2005 to show these awards.
2. The evidence of record also shows he was awarded the Army Achievement Medal by permanent orders on 24 June 2007. Therefore, based on this evidence, he is entitled to a correction of his DD Form 214 ending 5 May 2010 to shows this award and the Army Good Conduct Medal (1st Award) and Combat Action Badge.
3. In view of the foregoing, his records should be corrected as recommended below.
