THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her retirement orders and certificate to reflect her first and middle names as Michele Diane instead of Diane Michele" or "Diane M."
2. The applicant states that her retirement orders and her retirement certificate have her first and middle name reversed.
3. The applicant provides:
* Orders 10-293-00067, dated 20 October 2010
* her DD Form 363A (Certificate of Retirement), dated 19 November 2010
* A court order changing her name on 8 December 2009
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army (RA) on 20 August 1973 under the name of "Michele Diane A-----s." She was honorably discharged on 23 May 1976 for the purpose of immediate reenlistment. On 24 May 1976, she reenlisted in the RA. She continued to serve until she was honorably discharged on
2 November 1976. The DD Form 214 (Report of Separation from Active Duty) she was issued for this period of service shows her name as "Michele Diane
A-----s G----n."
2. On 13 October 1994, the applicant enlisted in the U.S. Army Reserve (USAR) for a period of 6 years in the rank/grade of specialist (SPC)/E-4. Her DD Form 4 (Enlistment/Reenlistment Document - Armed Forces of the United States) shows she enlisted under the name of "Michele Diane G----n."
3. Her interactive Personnel Enlisted Records Management File (iPERMS) file contains a DA Form 4187 (Personnel Action) that changed her name from "Michele D. G----n" to "Michele Diane."
4. On 24 September 2000, she reenlisted in the USAR for a period of 6 years. Her DD Form 4 for this reenlistment shows her name as "Michele Diane."
5. The applicant filed a Petition for Name Change through the Montana
4th Judicial District Court. On 8 December 2009, her name was legally changed from "Michele Diane" to "Michele Diane H------d."
6. Her iPERMS file contains an SGLV Form 8286 (Servicemembers' Group Life Insurance Election and Certificate) that shows her name as "Michele Diane
H------d." She designated as her principal beneficiaries her mother and father with the same last name of "H------d."
7. Orders 10-293-00067, issued by Headquarters, 88th Regional Support Command, Fort McCoy, WI, dated 20 October 2010, assigned the applicant to the Retired Reserve effective 19 November 2010, by reason of completion of
20 or more years of qualifying service for retired pay at age 60. Her name on these orders is reflected as "H------d Diane Michele."
8. Her DD Form 363A reflects her name as "Diane M. H------d."
DISCUSSION AND CONCLUSIONS:
1. The applicants contention has been noted and found to have merit. Her orders transferring her to the Retired Reserve and her retirement certificate incorrectly reflect her first and middle name as Diane Michele instead of Michele Diane.
2. Accordingly, Orders 10-293-00067 and her DD Form 363A should be corrected to show her correct name as "Michele Diane H------d" to replace the orders and certificate currently held by the applicant.

BOARD DETERMINATION/RECOMMENDATION:

a. amending Orders 10-293-00067 to show her name as "Michele Diane
H------d";
b. issuing the applicant a new DD Form 363A reflecting her first name and middle initial as Michele D. to replace the certificate currently held by her.

In this regard, the evidence of record shows that the applicant's military service records reflect the correct name (at the time) under which military service was performed.
