THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show:

* In block 7b. that his Home of Record (HOR) is spelled “Milledgeville”
* In block 8a that his last duty assignment was “020012ARRGTHHC TX FC”
* In block 13 that his NATO Medal was for Bosnia
* In block 13 that he was awarded the Meritorious Service Medal (MSM)
* In block 14 that he completed the Basic Noncommissioned Officer Course (BNCOC) Phase II for a period of 2 weeks in 2004

2.  The applicant states, in effect, that:

* His HOR is misspelled as “Middledgeville” instead of “Milledgeville”
* His last duty assignment was the 2d Battalion, 12th Armored Regiment and not the 3d Battalion, 67th Armored Regiment
* He received the NATO Medal for service in Bosnia and not Afghanistan as reflected on his DD Form 214
* He completed the BNCOC Phase II for a period of 2 weeks in 2004 and it is not reflected on his DD Form 214

3.  The applicant provides:

* A copy of his DD Form 214
* A copy of his Enlisted Record Brief (ERB)
* A copy of his NATO Certificate showing he was awarded the NATO Medal for service in Bosnia from March 1999 to August 1999
* A copy of his award of the MSM for his retirement on 31 August 2010
* A copy of his DA Form 1059 (Service School Academic Evaluation Report) showing his completion of the Fire Support Sergeant’s Course of 2 weeks duration in 2004
* A copy of his DA Form 1059 showing his completion of the BNCOC Phase I, 3 weeks in duration in 2008. 

CONSIDERATION OF EVIDENCE:

1.  The applicant’s HOR was in Milledgeville, Georgia when he enlisted in the Regular Army in Atlanta, Georgia on 3 January 1990 for a period of 4 years and training as a fire support specialist.  He completed his one-station unit training (OSUT) at Fort Sill, Oklahoma and he remained on active duty through a series of continuous reenlistments.  He was promoted to the pay grade of E-7 on 1 July 2008.

2.  On 31 August 2010, while assigned to Headquarters and Headquarters Company (HHC), 2d Battalion, 12th Armored Regiment, he was retired by reason of sufficient service for retirement.  He had served 20 years, 7 months, and       28 days of active service.  His DD Form 214 issued at the time of his retirement issued at the time of his retirement reflects in block 7b that his HOR was in “Middledgeville, Georgia” and in block 8a. that his last duty assignment was “HHC, 3d Battalion, 67th Armored Regiment.”  In block 13 it reflects that he was awarded the “NATO Afghanistan Service Medal” and does not reflect the MSM.  Block 14 does not contain his completion of the “Fire Support Sergeant’s Course” for a duration of 2 weeks in 2008.  

3.  The evidence of record shows that the applicant was awarded the NATO Medal for service in Bosnia.

4.  The evidence of record also shows that he was awarded the MSM at his retirement on 31 August 2010 and that he was assigned to the 2d Battalion, 12th Armored Cavalry Regiment at the time of his retirement (“020012ARREGT HHC FC”).

5.  A review of the DA Form 1059 for the Fire Support Sergeant’s Course clearly shows that the course is a combat arms skills training course.

6.  Army Regulation 635-5 serves as the authority for the preparation of the DD Form 214.  It provides, in pertinent part, that training courses for combat skills will not be entered on the DD Form 214.
DISCUSSION AND CONCLUSIONS:

1.  The applicant’s contention that his HOR is spelled incorrectly on his DD Form 214 has been noted and is found to have merit.  His DD Form 214 reflects that his HOR is in “Middledgeville, Georgia” when in fact it is in “Milledgeville, Georgia.”  Accordingly, his DD Form 214 should be corrected to reflect his proper HOR. 

2.  The applicant’s contention that his last duty assignment is incorrect has also been noted and found to have merit.  His DD Form 214 reflects his last assignment as “020067ARHHC TK BN (XX FC) when in fact it should reflect the entry “020012ARREGT HHC FC.”

3.  The applicant’s DD Form 214 incorrectly reflects that he was awarded the NATO AFGHANISTAN SERVICE MEDAL when in fact the evidence shows that it should be the “NATO MEDAL (BOSNIA).”  Additionally, the applicant’s award of the MSM should be added at this time.

4.  The applicant’s contention that his attendance at the 2-week Fire Support Sergeant’s Course in 2004 should be added to his DD Form 214 has been noted and found to lack merit.  That course is clearly a combat skills course and in accordance with the regulation, is not authorized for entry on the DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected:

* By deleting from block 7b of his DD Form 214 the line “Middledgeville, Georgia 31061"
* By adding to block 7b of his DD Form 214 the line “Milledgeville, Georgia 31061"
* By deleting the entry “030037ARHHC TK BN (XX FC) from block 8a of his DD Form 214
* By adding the entry “020012ARREGT HHC FC” in block 8a of his DD Form 214
* By deleting the entry “NATO AFGHANISTAN SERVICE MEDAL” from block 13 of his DD Form 214
* By adding the entry “NATO MEDAL (BOSNIA)” and the MSM to block 13 of his DD Form 214 

2.  The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.  As a result, the Board recommends denial of so much of the application that pertains to adding his completion of the 2-week Fire Support Sergeant’s Course in 2004 to his DD Form 214.  

3.  The Board wants the applicant and all others to know that the sacrifices he made in service to the United States during the Global War on Terrorism are deeply appreciated.  The applicant and all Americans should be justifiably proud of his service in arms.
