THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show all of his service in hostile fire/imminent danger pay (HFP/IDP) areas.
2. The applicant states his DD Form 214 does not reflect the time he served in combat nor does it reflect his other HFP/IDP tours. He claims the absence of this information affects his family and his health in relationship to his Department of Veterans Affairs (VA) claim.
3. The applicant provides a copy of his Enlisted Record Brief (ERB).
CONSIDERATION OF EVIDENCE:
1. The applicants record shows he served on active duty for 20 years and
4 days until being honorably retired on 31 May 2010 in the rank of master sergeant (MSG)/E-8.
2. The applicants ERB shows he served in the following overseas areas for the periods and during the dates indicated:
* Cuba, 11 months, 8 May 2007 - 8 April 2008
* Korea, 12 months, 18 February 2000 - 8 February 2001
* Haiti, 4 months, 26 February 1995 - 13 June 1995
* Saudi Arabia, 5 months, 7 December 1990 - 26 April 1991
* Germany, 21 months, 1 November 1990 - 9 July 1992
3. The applicants record also shows he earned the following awards during his military service:

4. Item 18 (Remarks) of the applicant's DD Form 214 issued to him upon his retirement on 31 May 2010 contains an entry indicating he served in the IDP area of Cuba from 8 May 2007 - 8 April 2008. No other HFP/IDP or deployed areas of service are listed.
5. Army Regulation 635-5 (Personnel Separations - Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states that for an active duty Soldier deployed to a foreign country with his or her unit during their continuous period of active service, the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)" will be entered in block 18.
DISCUSSION AND CONCLUSIONS:
The applicants contention that his deployed combat service and service in IDP/HFP areas should be added to his DD Form 214 has been carefully considered and found to have merit. His record confirms that in addition to being deployed to Cuba, the applicant was deployed to Saudi Arabia from 7 December 1990 through 26 April 1991 and to Haiti from 26 February through 13 June 1995. Therefore, it would be appropriate to correct his DD Form 214 by documenting these deployments in item 18 of his DD Form 214.

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to item 18 of his DD Form214 for the period ending 31 May 2010 the entries "SERVICE IN SAUDI ARABIA (SOUTHWEST ASIA) FROM 19901207-19910425 and SERVICE IN HAITI FROM 19950226-19950613.
