THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of his third Meritorious Service Medal and his second Army Reserve Components Overseas Training Ribbon.

2.  The applicant states that these awards were left off of his DD Form 214.

3.  The applicant provides copies of his certificate for award of the Meritorious Service Medal and two letters indicating he was twice awarded the Army Reserve Components Overseas Training Ribbon in support of his application.

CONSIDERATION OF EVIDENCE:

1.  In 1985, the applicant enlisted in the U.S. Army Reserve and was subsequently ordered to active duty.

2.  Permanent Orders 45-09, dated 21 July 1994, awarded the applicant the Meritorious Service Medal for exceptionally meritorious service while assigned to the 96th U.S. Army Reserve Command from 15 April 1992 to 1 August 1994.

3.  A memorandum from the 330th Combat Support Hospital, dated 7 April 2000, indicates the applicant was awarded the Army Reserve Components Overseas Training Ribbon for completion of annual training in Paraguay from 17 March 2000 to 1 April 2000.

4.  Permanent Orders 242-11, U.S. Army Human Resources Command, Alexandria, Virginia, dated 30 August 2006, awarded the applicant the Meritorious Service Medal for outstanding meritorious service as Chief, Executive Services, U.S. Army Human Resources Command, St. Louis, Missouri, from 6 November 2003 to 7 January 2007.

5.  A memorandum from the 9th Mission Support Command, dated 14 May 2008, indicates the applicant was awarded the Army Reserve Components Overseas Training Ribbon for participation in Exercise Garuda Shield 2008 in Jakarta, Indonesia, from 26 March 2008 to 13 April 2008.

6.  On 30 April 2009, the applicant was retired due to sufficient service.  He had attained the rank of sergeant first class/pay grade E-7 and had completed a total of 21 years, 1 month, and 25 days of creditable active duty service.

7.  Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) of the applicant's DD Form 214 shows only two awards of the Meritorious Service Medal and one award of the Army Reserve Components Overseas Training Ribbon.

8.  Permanent Orders 212-06, 9th Mission Support Command, dated 31 July 2009, awarded the applicant the Meritorious Service Medal for exceptionally meritorious service in successive positions of increasing responsibility from 9 January 1999 to 1 May 2009.

9.  Army Regulation 635-5 (Separation Documents) provides detailed instructions for completing separation documents, including the DD Form 214.  Item 13 of the DD Form 214 provides for the entry of all decorations, service medals, campaign credits, and badges awarded or authorized during all periods of service.

DISCUSSION AND CONCLUSIONS:

1.  The applicant contends that his DD Form 214 should be corrected to show award of his third Meritorious Service Medal and his second Army Reserve Components Overseas Training Ribbon.

2.  The available evidence clearly shows the applicant was awarded three awards of the Meritorious Service Medal and two awards of the Army Reserve Components Overseas Training Ribbon.

3.  In view of the above, the applicant's request to add his missing awards should be granted.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned, in particular his DD Form 214 for the period ending 30 April 2009, be corrected by:

	a.  deleting from item 13:  Meritorious Service Medal (2nd Award) and Army Reserve Components Overseas Training Ribbon and

	b.  adding to item 13:  Meritorious Service Medal (3rd Award) and Army Reserve Components Overseas Training Ribbon (2nd Award).
