THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show all of her enlisted service.
2. The applicant states that her DD Form 214 does not reflect all of her enlisted service.
3. The applicant provides, in support of her application, copies of her DD Form 214, DD Form 4 (Enlistment/Reenlistment Document), DA Form 1506 (Statement of Service), orders for initial active duty for training (IADT), memorandum for appointment as an Army Reserve Officers' Training Corps cadet, discharge orders, and her DA Form 2-1 (Personnel Qualification Record - Part II).
CONSIDERATION OF EVIDENCE:
1. On 8 September 1989, the applicant enlisted in the U.S. Army Reserve (USAR). She served in an inactive duty status until 6 June 1990 (8 months and 29 days).
2. The applicant was ordered to IADT during the period from 7 June through
10 August 1990 (2 months and 4 days).
3. The applicant served in an inactive duty status during the period
11 August 1990 through 16 July 1991 (11 months and 5 days). She was advanced to private, pay grade E-2, effective 7 September 1990.
4. The applicant served in an active duty status during the period 17 July through 19 August 1991 (1 month and 3 days).
5. The applicant served in an inactive duty status during the period 20 through 28 August 1991 (9 days). She was discharged, effective 28 August 1991 for the purpose of enrollment in the Reserve Officers' Training Corps (ROTC).
6. The applicant was enrolled as an ROTC cadet during the period 29 August 1991 through 14 July 1994.
7. The applicant was ordered to active duty, effective 15 July 1994, as a second lieutenant, pay grade O-1, Chemical Corps.
8. On 14 August 2007, the applicant was discharged. She had completed
13 years and 1 month of creditable active service during this period.
9. Block 12 (Record of Service) of the applicant's DD Form 214 shows no prior active or inactive service.
10. Army Regulation 635-5 (Separation Documents) provides detailed instructions for completing separation documents, including the DD Form 214. It provides, in pertinent part, that the individual's record of service will be recorded in Block 12 (Record of Service).
a. For item 12a (Date Entered Active Duty (AD) This Period), enter the beginning date of the continuous period of AD for issuance of this DD Form 214, for which a DD Form 214 was not previously issued.
b. For item 12b (Separation Date This Period), enter the Soldier's transition date. This date may differ from the contractual date if the Soldier is separated early, voluntarily extends, or is extended due to lost time, or retained on active duty for the convenience of the Government.
c. For item 12c (Net Active Service This Period), enter the amount of service this period, computed by subtracting item 12a from 12b. Lost time under Title 10, U.S. Code, section 972 and non-creditable time after expiration term of service (ETS), if any, are deducted. Such time will be identified in item 18 (Remarks).
d. For items 12d (Total Prior Active Service) and 12e (Total Prior Inactive Service) enter the total amount of prior active and inactive military service. If not applicable, enter all zeros.
DISCUSSION AND CONCLUSIONS:
1. The applicant contends that her DD Form 214 should be corrected to show all of her prior enlisted service.
2. The available evidence shows the applicant served a total of 3 months and
7 days of prior active duty service. She also served a total of 1 year, 8 months, and 13 days of prior inactive service. Her DD Form 214 should be corrected to show this service.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. deleting the entry in Item 12d of the applicant's DD Form 214 and adding
3 months and 7 days; and
b. deleting the entry in Item 12e of the applicant's DD Form 214 and adding
1 year, 8 months and 13 days.
