THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 11 July 2003 be added to his Official Military Personnel File (OMPF).

2.  The applicant states that his DD Form 214 for the period ending 11 July 2003 for his deployment in support of Operation Enduring Freedom has not been included in his permanent record.

3.  The applicant provides a copy of his DD Form 214 for the period ending 
11 July 2003 and orders in support of his application.

CONSIDERATION OF EVIDENCE:

1.  Having prior inactive commissioned service in the U.S. Army Reserve, the applicant was ordered to active duty on 20 March 2003 in support of Operation Enduring Freedom.  He served as a field surgeon in Germany from 30 March 2003 to 29 June 2003 and was released from active duty on 11 July 2003 in the rank of lieutenant colonel.         

2.  The applicant's DD Form 214 for the period ending 11 July 2003 is not shown on his OMPF on the interactive Personnel Electronic Records Management System (iPERMS).  


3.  Army Regulation 600-8-104 (Military Personnel Information Management/ Records) prescribes the policies governing the OMPF, the Military Personnel Records Jacket, the Career Management Individual File, and Army Personnel Qualification Records.  

4.  Table 2 of Army Regulation 600-8-104 states, in pertinent part, that DA Forms 214 will be filed in the service section of the OMPF.

DISCUSSION AND CONCLUSIONS:

Since the governing regulation states that DD Forms 214 will be filed in the service section of the OMPF, and the applicant's DD Form 214 for the period ending 11 July 2003 is not shown on his OMPF, it would be appropriate to add the DD Form 214 for the period ending 11 July 2003 on his OMPF.
