THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show foreign service in block 12f and the Army Commendation Medal as a authorized award.
2. The applicant states the requested information is missing from his DD Form 214.
3. The applicant provides his DD Form 214 and the certificate for his Army Commendation Medal.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 13 September 2005, completed training as an armor crewman, and was stationed in Germany on 18 January 2006.
2. He deployed with his unit to Iraq on 28 August 2006 where he was wounded on 30 June 2007 and advanced to specialist, pay grade E-4 on 8 September 2007.
3. He returned to Germany on 13 November 2007 and to the United States on 12 March 2008.
4. The applicant was awarded the Army Good Conduct Medal for the period
8 September 1205 to 7 September 2008 by Headquarters, U.S. Army Armor Center and Fort Knox Permanent Order Number 241-0053, dated 28 August
2008.
5. On 10 April 2009, the applicant was separated with a general discharge.
6. Block 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) lists the Purple Heart, National Defense Service Medal, Iraq Campaign Medal with Campaign Star, Army Service Ribbon, Overseas Service Ribbon (2nd Award), and the Combat Action Badge. Block 12f shows no foreign service.
7. His Enlisted Record Brief lists the same awards.
8. The certificate submitted by the applicant shows that Headquarters, 2nd Brigade, 1st Infantry Division Permanent Orders Number 114-011, dated 24 April 2007, awarded him the Army Commendation Medal for meritorious service in Iraq.
9. Army Regulation 600-8-22 (Military Awards) states the Global War on Terrorism Service Medal (GWOTSM) is authorized for award to members of the Armed Forces of the United States who have participated in the Global War on Terrorism operations outside of the designated areas of eligibility (AOE) on or after 11 September 2001 to a future date to be determined. All Soldiers on active duty, including Reserve Component Soldiers mobilized or National Guard Soldiers activated, on or after 11 September 2001 to a date to be determined having served 30 consecutive days or 60 nonconsecutive days are authorized the GWOTSM.
DISCUSSION AND CONCLUSIONS:
1. The applicant states that block 12f of his DD Form 214 should show his foreign service and block 13 should list the Army Commendation Medal.
2. The applicant served overseas from 18 January 2006 through 11 March 2008, a period of 2 years, 1 month, and 24 days. This should be shown on block 12f of his DD Form 214.
3. The applicant was awarded the Army Commendation Medal by Headquarters, 2nd Brigade, 1st Infantry Division Permanent Orders Number 114-011, dated
24 April 2007, and the Army Good Conduct Medal by Headquarters, U.S. Army Armor Center and Fort Knox Permanent Order Number 241-0053, dated 28 August 2008. These awards should be added to his DD Form 214.
4. The applicant also met the eligibility criteria for award of the GWOTSM, and it should be added to his DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. showing "0002 01 24" in block 12f of his DD Form 214; and
b. showing that, in addition to the awards listed on his DD Form 214, he is also authorized the Army Commendation Medal, the Army Good Conduct Medal, and the Global War on Terrorism Service Medal.
