THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to add his second Bronze Star Medal.
2. The applicant makes no additional statement.
3. The applicant provides, in support of his application, copies of his DD Forms 214 ending on 8 April 2005 and on 11 January 2009; a Bronze Star Medal Certificate; and a DA Form 638 (Recommendation for Award) initiated on
19 November 2008.
CONSIDERATION OF EVIDENCE:
1. At the time of his application, the applicant was serving in the United States Army Reserve in the rank of lieutenant colonel, pay grade O-5.
2. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of the applicants DD Form 214, ending on 8 April 2005, lists his awards as the Bronze Star Medal, Meritorious Service Medal, Army Commendation Medal, Army Reserve Components Achievement Medal
(6th Award), National Defense Service Medal (2nd Award), Global War on Terrorism Expeditionary Medal, Korean Defense Service Medal, Global War on Terrorism Service Medal, Armed Forces Expeditionary Medal, Armed Forces Reserve Medal with "M" Device, Armed Forces Reserve Medal with Ten-Year Device, Overseas Service Ribbon, Army Reserve Components Overseas Training Ribbon, Parachutist Badge and the Air Assault Badge.
3. A DA Form 638, initiated on 19 November 2008, recommended the applicant for award of the Bronze Star Medal for meritorious service for the period from
19 January to 11 December 2008 while assigned to the Multi-National Force-Iraq. Item 8 (Previous Awards) of this form indicates that the applicant was previously awarded the Bronze Star Medal.
4. On 23 December 2008, the appropriate authority approved the award recommendation. Permanent Order Number 337-049, dated 2 December 2008, awarded the Bronze Star Medal [the second award] to the applicant.
5. On 11 January 2009, the applicant was released from active duty. He had completed 1 year, and 5 days of creditable active service during this period of active duty.
6. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of the applicants DD Form 214, ending on 11 January 2009, lists his awards as the Iraq Campaign Medal, Bronze Star Medal, Meritorious Service Medal, Army Commendation Medal, Army Reserve Components Achievement Medal (6th Award), National Defense Service Medal (2nd Award), Armed Forces Expeditionary Medal, Global War on Terrorism Expeditionary Medal, Global War on Terrorism Service Medal, Korean Defense Service Medal, Overseas Service Ribbon (2nd Award), Army Reserve Components Overseas Training Ribbon, Armed Forces Reserve Medal with "M" Device, Armed Forces Reserve Medal with Ten-Year Device, North Atlantic Treaty Organization Medal, Parachutist Badge and the Air Assault Badge. It does not show award of the second Bronze Star Medal.
7. Army Regulation 600-8-22 (Military Awards) provides, in pertinent part, that an Oak Leaf Cluster is issued to denote award of the second and succeeding awards of the Bronze Star Medal.
8. Army Regulation 600-8-22 (Military Awards) provides, in pertinent part, for the wear of a bronze service star on the Iraq Campaign Medal for service during any of the following campaign periods: Liberation of Iraq from 19 March 2003 to
1 May 2003; Transition of Iraq from 2 MAY 2003 to 28 June 2004; Iraqi Governance from 29 June 2004 to 15 December 2005; National Resolution from16 December 2005 to a date to be determined.
DISCUSSION AND CONCLUSIONS:
1. The applicant contends that his DD Form 214 ending on 11 January 2009 should show award of a second Bronze Star Medal.
2. The available evidence shows the applicant was awarded a Bronze Star Medal sometime prior to 8 April 2005 and again on 2 December 2008. Therefore, his DD Form 214, ending on 11 January 2009, should be corrected to show both of these awards.
3. Furthermore, the evidence shows that the applicant received the Iraq Campaign Medal indicating that he participated in at least one campaign in Iraq. Therefore, he should be authorized to wear a bronze service star on his Iraq Campaign Medal.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. deleting the Bronze Star Medal from his DD Form 214 ending on
11 January 2009; and
b. adding to that same DD Form 214 the Bronze Star Medal with Oak Leaf Cluster and a bronze service star to be affixed to his already-awarded Iraq Campaign Medal.
