THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to reflect his awards of the Army Commendation Medal (ARCOM) and the Joint Service Achievement Medal (JSAM).

2.  The applicant states that the awards were not available at the time his DD Form 214 was prepared and thus were omitted.  Accordingly, he desires them to be added at this time. 

3.  The applicant provides a copy of his DD Form 214 and copies of certificates and orders awarding him the ARCOM with two oak leaf clusters (ARCOM 2OLC) and the JSAM.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army (RA) on 4 October 2001 for a period of 4 years, training as a preventative medicine specialist, a cash enlistment bonus, and assignment to the continental United States for his first assignment.

2.  He completed his training at Fort Sam Houston, Texas and was awarded military occupational specialty (MOS) 68S (preventative medicine specialist.  He was transferred to Aberdeen Proving Ground, Maryland for his initial duty assignment.  He was advanced to the pay grade of E-4 on 1 July 2003.
3.  On 8 December 2004, he reenlisted for a period of 4 years, assignment to Hawaii, and a selective reenlistment bonus.

4.  He was subsequently transferred to Hawaii and was promoted to the pay grade of E-5 on 1 March 2006.

5.  The applicant was awarded the ARCOM 2OLC for meritorious service during the period from 16 April 2005 to 7 December 2008, by Tripler Army Medical Center, Hawaii, Permanent Orders Number 315-022, dated 10 November 2008.

6.  The applicant was awarded the JSAM for meritorious service during the period from April to October 2008, by Headquarters, Joint Task Force – Bravo Permanent Order 317-01-08, dated 12 November 2008.

7.  On 7 December 2008, the applicant was honorably released from active duty (REFRAD) due to completion of required service.  He had served 7 years, 2 months, and 4 days of total active service.  Neither the ARCOM 2OLC or the JSAM were entered in item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 at the time of his REFRAD.

DISCUSSION AND CONCLUSIONS:

1.  The applicant was awarded the ARCOM 2OLC and the JSAM shortly before his REFRAD; however, for unknown reasons, those awards were not added to his DD Form 214 at the time of his REFRAD.

2.  Therefore, it would be in the interest of justice at this time to correct his records to show that he was awarded the ARCOM 2OLC and the JSAM.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to item 13 of the applicant's DD Form 214 the Army Commendation Medal with 2nd oak leaf cluster and the Joint Service Achievement Medal.

2.  The Board wants the applicant and all others to know that the sacrifices he made in service to the United States during the Global War on Terrorism are deeply appreciated.  The applicant and all Americans should be justifiably proud of his service in arms.
